>> SARAH: We have a traditional system of caring for people that is really based on
caring for people who are essentially well. You know, most people are healthy, but we
also have an aging population with a lot of chronic diseases, and to care effectively
for those patients we have to come up with new systems for caring for them because the
old model doesn't work. Our post-hospital discharge clinic is the most obvious innovation
we've made. We've introduced a clinic that provides hour-long appointments with a nurse
practitioner within 72 hours of discharge from the hospital, and we're able to capture
patients before they're too far out from discharge and catch what we call the low-hanging fruit
– problems with medications, questions about their disease process, problems with making
appointments - and address those things and help people stay at home. I think the benefit
to patients is that in our post-hospital discharge clinic we spend a lot more time with them
and they really appreciate the opportunity to ask questions, they appreciate that we
do a more thorough assessment, we don't just ask about their health problems - we ask about
their life. And I think that holistic perspective changes the way patients look at healthcare.
