and their improvement or not, and I can change the management.
And I feel like, you know, I'm easily accessible by my patient, the family member,
their physician because I will be available about the whole thing.
So we helps our patient to recover quickly from acute illness because they need
to be hospitalized because acute illness.
And when they recover, and when they ready to be discharge,
I transfer my care back to their primary care physician.
And I strongly believe in excellence in transition of care,
and whenever my patient are ready to be discharged, you know, my, if my patients
or family member ask me to call the doctor, their primary care provider or I believe I need
to call their primary care doctor in person, I give them a call
and discuss [inaudible] treatment plan to rehospitalization and after the charge.
When they ready to go home, you know, they smile, and they, you know.
So this is the time that I enjoy the most when people get better and ready to be discharge.
