Interpreter: It is unlawful for an owner, operator, or employee of a
place of public accommodation to refuse or deny any of the advantages,
facilities, and privileges of that place because of a person’s hearing loss.
You must file a complaint within 6 months of the alleged unlawful incident
in the case of discrimination by a place of public accommodation.
There are three ways to file a report with MCHR.
First, by using MCHR’s online form.
Second, by calling MCHR at their toll-free number

or Third, by writing a letter to MCHR.
Using the Online form is quick and easy.
Visit the MCHR website and click on the “Public Accommodations,”
which is on the left side of the screen under the heading “Our Services.”
Then click on the link for the online form.
You will notice that some questions are identified with an asterisk (*),
which indicates a mandatory field that must be answered
prior to submitting the form.
You will be required to provide your name and address,
the name and address of the place that discriminated against you,
and a brief description of what happened.
It is important to note that completing the online form
does NOT constitute the filing of a complaint of discrimination
with MCHR.
For your complaint to be valid, you must send MCHR
a written complaint with your signature.
Because you will have to send MCHR a written complaint anyway,
you may want to go ahead and send them a letter.
If you decide to write a letter, you must include at least the following:
Your name and address, the name and address of the company
your complaint is about, the name of the City or County
the violation occurred, the date when this incident occurred,
a short description of what happened, and
you must remember to sign your letter!
