I'm going to give you an overview of what a
post high school educational transition goal is
We will cover the following topics:
Introduction to the higher educational goal
How to help determine a career goal if you don't already know one
Ways to find out about the education requirements needed for your career choice
Your next steps once you have the information you need to prepare for your career

Checklist for self-advocacy and the
Next steps to prepare for your future
What is your plan after high school?
Transition is defined as a movement, passage, or change from one position, stage, subject,
concept, etc, to another;
or, change: the transition from adolescence to adulthood
As a team, we are going to help you transition into higher learning
to help prepare you for your career
What kind of career are you interested in?
If you don't already know, take the career interest inventory at California Career Zone
I will give you the log in information
If you already know the career that interests you, California Career Zone
will give you additional information about the types of jobs
within the employment field of your choice,
the requirements needed, and the future outlook for this type of employment
Think about the career you choose as one you will find rewarding, interesting,
and you are willing to meet the demands of preparing for and maintaining this career
Once you have decided on and researched your career choice,
find out which higher education institutions
offer the programs you will need to reach your employment goal
Your next steps are to:
Research the admission requirements for the program of your choice
Identify the high school classes you need to prepare for your career interest
Visit the campus of your choice; while there, gather information; identify related services available to you
Advocate for your educational needs at your IEP meetings
Identify people who will be positive mentors
Choose community services that will provide experience toward your career choice
As your special education case manager, I and the rest of your IEP team,
will guide you with these steps
When you are within one year of applying for admission to a higher learning institution,
whether it be college, ROP, or a trade school, find out about the
financial aid packages available to you
You can go online and search, ask your school counselor,
and/or contact your school choice directly
There are many scholarships offered for students with various disabilities
I can help you search for these
Each federally and state funded educational institution will have a
student disability support office
To continue with support servcies, you will need to:
Visit the student disability support office
Find out what documentation is required to attain support services
Inquire about the accommodation request procedure
and of course, follow through with the requirements
By now, you hopefully know which accommodations help you succeed in your academics
Be prepared to explain to the staff at the Student Disability Support Office
your learning style and the appropriate accommodations you will need
You will have support with your disability once you enter a higher learning institution;
however,
You will be responsible for making sure all paperwork is completed
and that all deadlines are met
I added a check list for self-advocacy skills from a transition booklet called
"Catching the Wave"
provided through a combined effort of the San Diego Community Colleges
I would recommend downloading a copy for yourself
Not everything in the transition booklet will apply to you, but it gives you
a general idea about what is needed
to obtain support for your disability
when you are preparing to leave high school
Pause the video here and read the "Steps to Self-Advocacy" aloud
Then we will discuss your thoughts on self-advocacy before restarting the video
