To access Dash N Doc its very simple. Go to the web, type in DashNDoc.com. Go to the green
connect button. This will take you to the login page. Select consumer, and it is very
easy to enroll. You just need your name, date of birth, and some simple information. You
will receive a confirmation email back which will allow you to link right back into your
account. Remember this is your electronic medical records account, and you will be able
to keep your records, your medicines, and any procedures or images you want saved. These
records can even be exported to your primary care physician, a hospital, or an emergency
room if needed.

Dash-N-Doc is a portal into the healthcare system. We encourage you to have a primary
