To fill out your China visa application form in this video.
First step please go to China
visa application center website at www.chinavisaapplication.us dot
then choose the correct type of visa that applies to you
different types of visa have different requirements
so please make sure you choose the right type of visa for your needs
for me I will click China tourist visa
you will then be directed to a link which contains detailed information
about what documents you will need for obtaining a tourist visa
click on China visa application form
to open the PDF form in a new browser window
please note: if you are applying for work visa
student visa if you are non-us citizen
you also need to fill out the China visa supplementary form
the detailed video instructions on how to fill out the supplementary form is on
the second part of
this tutorial video so after clicking the China visa application form link
we are brought to the provided form okay
assuming you're good to go let's begin. The form must be filled out completely
all information must be typed in capital English letters in a box.
If you do not have an answer to a question or question does not apply to
you
please type in an "N/A" instead of leaving it blank
the Chinese consulate does not accept hand written documents
the only party should hand right is the signature
after printing out the completed form. In box 1.1
there asking for your full name surname is your last name
and given name is your first name plus middle name
for example if your name is John Franklin
you should put Franklin in the box for your surname
and put John for your given name please make sure that your name on this form
exactly matches your name on your passport inbox 1.2
please choose your sex male or female
inbox 1.3
if you are poor in mainland China Taiwan or Hong Kong
you must put your full Chinese name in Chinese characters
otherwise please write down an "N/A"

put down other names you have to have been known by

put your name and ethnic script if you're English name is translated from
another language
such as Korean or Japanese, put your name in native language here.

Chinese people prefer writing birth dates starting with the year
then the month ending in the date with a dash between them

for example I was born in Houston
so I will put down Houston Texas

local ID card number if you have a driver's license
please put on your state's driver's license number
if not you may also put down your passport number.
However driver's license number is preferred.
In 1.12 choose your marital status.
For 1.13 click your current occupations.
If your occupation is not listed here you may click
other and type in your current occupation
for box 1.14 please choose your passport type
if you are not going as a government official like diplomatic person
click on ordinary.
For box 1.15 place down your passport number
US passport has nine digits containing numbers or letters.
It's on the upper right corner of your passports information page.
In box 1.16 put the date
that your passport was issued. This information is in your passport.
Please put down the year first following with month
ending with date.
Inbox 1.17 Fill in the place where your passport was issued.
You may just put down US a if you have a US passport.
In box 1.18 put your passport expiration date,
starting with the year then the month
and lastly the day. In box 1.19
please choose your processing time, note that faster processing will require
extra fees.
In my case I will choose regular
Now for section 2 your travel information,
In box 2.1, please choose your visiting purposes,
this section is really really important for determining your visa type
by the Chinese consulate. I will choose
tourism because I'm applying for tourist visa.
Please do not use business trip meeting
or any other business-related travel purposes for tourist visa.
Box 2.2 is your intended number of entries into China.
If you are planning to enter china and 24 months
you can also specify by type in multiple
entries valid for 24 months. Please note:
24 months Visa is not available for most applicants
if you must have a 24 month visa please attach
a letter to explain the reason.
Box 2.3 you enter the date of your first
entry to China beginning with the year
then the month and finally the day.
Box 2.4 for longest intended stay in China
this question self-explanatory, put
in the number of days in the box.
For box 2.5 put down your residence information and China
for example if you're staying at the Hilton and Shanghai
you should put down Hilton Hotel then the address
one tingly highway Shanghai
and the phone number at the place which is 21
62480000.
If you're staying at a friend's home please put on your friend's home address.
The important thing is that you must include the name of the city in the
address.
In box 2.6 please put down who will pay for the cost of the Traveling
while living in China. In box 2.7
if you have a medical insurance that covers for international travel to China
please put down yes and the company number
following with insurance account number. If you don't have insurance
just put down no.
In box 2.8 place but down your inviter's information.
If you don't have an inviter you can simply put
a "N/A". In box 2.9
if you are visiting friends or family in China,
write down information in the box, if not
write "N/A".
Form section 3 information about your family
work or study inbox
3.1 fill out your details: home address
this should be the same address as your address on the proof of residence
document,
such as photocopy of your drivers license for utility bill.
In box 3.2 please put down your home phone number.
In box 3.3 please put down your cell phone number.
In box 3.4 please put down your email address.
In box 3.5 put your current employer or school information,
including name and the phone number.
You can also put self-employed and slash
a or unemployed if this applies to you.
In box 3.6 major family members,
these are your spouse and children information.
You may also put down your parents information if you wish.
In box 3.7 please put down your emergency contact information.
next section 4 other information.
In box 4.1 have you ever visit to China before?
If no put down no, if yes
but down yes and type in the description of your last visit.
In box 4.2 have you visited other countries
in the last 12 months? I will put down no
because I haven't. you need to put down yes,
if you have visited other countries in the past 12 months,
because your travel information should have been recorded your passport before.
Always type truthful information on the application form.
Box 4.32 box 4.7 are yes or no questions that you need to answer carefully.
Choose the options that apply to you.
If you answered yes to any of these questions,
please also note down the details in box 4.8.
Otherwise leave the box blank.
next section 5 we recommend you to leave box
5.2 blank unless you have something important to say about your application.
The final step to sign and date your application in section 6.
Please leave section 7 blank unless someone else
other than the applicant fills out the form.
Once you're done, print out your form, gather
all other required documents and mail them to China visa application center.
