Medicare does not generally cover long-term care provided at home, in the community, or
in nursing homes.
Most long-term care such as help with the activities of daily living, is considered
"custodial care"... and is not covered by Medicare.
Furthermore, Medicare only pays for medically necessary skilled nursing facility care.
