What do I mail to USCIS?
You must mail all of the following to USCIS.
First, your completed and signed Form I-765, Application for Employment Authorization
Second, your completed and signed form G-1145, E-Notification of Application/Petition Acceptance.
Third, your check or money order for $380 payable to "U.S. Department of Homeland Security."
If your name and address (as listed on the I-765) is not pre-printed on a personal check,
then you must get a money order instead.
Your 2 passport style, color photos in a small envelope. On the back of the photos, print
your full name, date of birth, I-94 number and SEVIS ID number which is printed on the
top right corner of your I-20.
Review the photo specifications needed by visiting the link listed.
Copy of your new I-20 form with the OPT recommendation. Be sure to sign on the bottom of page 1 of
your new I-20 before you photocopy.
Copies of ALL your previous I-20 forms, from NYU and previous schools.
If you have lost any previous I-20 contact your previous school's international student
office. If there is no copy on file, write a letter detailing where, when and at what
degree level you attended school as an F-1 student, whether you received OPT, and explaining
why you cannot provide a copy of your I-20. Be sure to include your name on this letter.
Copy of your I-94 record. For details on this, visit the link shown.
Copy of your F-1 visa stamp. Canadian citizens are exempt from this.
Copy of your passport photo page. Include a copy of your passport expiration date page
if it is separate from the photo page
Finally, photocopy the contents of your OPT application once you have compiled them and
keep them for your records.
Send your complete application to USCIS. If the address you listed on the I-765 is in
New York or in the Northeast, send your application using a private courier service (for example
UPS or FedEx) only to the listed address.
If the address on Form I-765 is in a different part of the United States, ask the OGS to
confirm the mailing address you should use. Step two is now complete!
