Treatment of vocal fold scar is very difficult.
It's one of the hardest things for us to, uh,
treat and improve,
so we'll start usually with something conservative.
We'll often do a course of voice therapy,
um, which may or may not loosen up the scar tissue
ever so slightly, but more often than not,
will give a patient different ways
of accessing parts of their range,
or, um, colors of the voice that might be missing.
If that's not successful, we can intervene surgically.
Um, again, it's difficult.
You can push over the vocal fold
so it closes better against the other one
by augmenting it with injections or implants.
Or you can address the scar directly.
You can do it in many different ways,
because there's not one great way
that everyone agrees that is the best.
So you can just raise it up,
you can cut into it,
you can inject it with steroids,
you can laser it.
You can put a fascial graft underneath the scar
to try and lift it up and allow it to vibrate again.
