Grace, congratulations, welcome to the program, good to have you
part of the Best Docs Network. Thank you, Jim, IÄôm so excited
to be here and we have a great show for you today. WÄôre
going to start by talking about some minimally invasive laser
procedures that can help with back pain. I know we all
experience some of that. Definitely. So leÄôs go ahead
and take a look, itÄôs with pain management specialist Dr.
Jason Williams. Well Jesse came to me complaining of low back
pain and leg pain. It had been plaguing him for several years
and been progressively getting worse. It was to the point where
it was affecting his job. Every time I had a back spasm you know
IÄôd be out for 2 or 3 days where my back would hunch over
and IÄôd be stuck in this weird, awkward sideways position
hunched sideways and every night when I went to sleep, I dreaded
going to sleep because I knew IÄôd wake up in pain. Sure
enough it was a bulging disc on L4 and L5. Well initially my
doctor referred me to a spine specialist that he was familiar
with and comfortable with so I saw him for a while. I went in
for cortisone injections, went through that, went through
physical therapy and all of it helped you know it did help
alleviate some of the pain but it eventually came back so the
doctor recommended actually having some type of invasive
surgery and it was something I really wasnÄôt comfortable
with. Luckily at that time Dr. Williams told me about the
accurascope procedure. The accurascope procedure is a
minimally invasive spine surgery that addresses conditions of the
spine including herniated discs, bulging discs, degenerative
discs and or scar tissue in a patientÄôs spine. The way he
presented it was, here are the facts, hereÄôs whaÄôs been
done on other patients and herÄôs the proven success
rate. You know oftentimes patients come to me you know and
theÄôre in so much pain and have their extremities affected
so much that they canÄôt walk without using assistive device,
that be a wheelchair, a cane, a walker. Upon doing the
accurascope procedure a lot of these patients no longer need an
assistive device. When I woke up from the surgery I knew that it
worked you know and there was this weird feeling of intense
pressure gone from my back. After Jesse had the accurascope
procedure he followed up with me and I determined that JesseÄôs
pain was 100 percent gone. I have now been able to focus more
on work, focus more on enjoying my family, getting just good
quality sleep. Äôve been pain free for 9 months now. Hi and
welcome back to the Best Docs Network In the Studio. WeÄôre
joined today by Dr. Harvey Carter, founder of Carter Eye
Center and one of the top lasik surgeons in not only the Dallas,
Fort Worth area but in the country. ItÄôs a privilege to
have you here Dr. Carter, thanks for joining us. Thank you for
having me. Now you were the first FDA investigator to
perform lasik surgery in north Texas, so youÄôre obviously an
expert on the subject matter so IÄôd like you to talk to us
just a little bit about lasik surgery. If you would start off
just by taking us through the process. The process today is
really just dramatic in terms of iÄôs ability to rapidly change
someoneÄôs life in a matter of minutes. TodaÄôs technology is
blade free, all laser, laser vision correction. On the
surgery day, the patient comes in and they sit in a very
comfortable room where their eye drops are applied. They get a
light sedation with *** or something like that, they want
chemical courage as iÄôs called. They then go in the
laser vision correction room and in that room we have a laser
that makes your flap, so you have a flap made in your cornea.
We have another laser that takes your personalized prescription
from your eye and lasers that prescription directly on to your
eye. If your prescription was whatever it was in your glasses
or contact lenses, it can be literally sculpted on to your
cornea so you can throw away hopefully your glasses or
contact lenses. You put the flap back and they go out and the
entire procedure from walking in the room to walking out of the
room is usually less than 5 minutes. Oh wow, thatÄôs
amazing. IsÄôt it great? Absolutely. And if a patient has
lasik surgery, will they be glasses free for the rest of
their life or is it possible that their eyes will regress and
theÄôll have to have surgery again? There are a variety of
ways of doing lasik surgery. Lasik in general will treat
nearsightedness, farsightedness, astigmatism, so it will
basically reduce or eliminate the need for glasses for anyone

you want to have decent hearing when yoÄôre in your 90Äôs you

most common complications that wÄôre trying to avoid is
capsular contracture. What that means is when you put in an
artificial device in any person whether it be a breast implant,
a pace maker or an artificial joint is the body makes a layer
of tissue around it called a capsule. That happens in the
first week and thatÄôs the patients own tissue and thaÄôs
a normal response. Unfortunately in a minority of patients that
capsule can start to contract down and harden so the breast
implant instead of resting on the chest like a dome shape
becomes rounded pulls up distorted and can become
painful. After breast augmentation there are certain
measures that are employed but to try to prevent capsular
contracture commonly is to massage the breast implant
daily. Nevertheless probably about 20 percent of patients
over the next 5 to 10 years are going to get that hardening. Now
traditionally iÄôs been taught that the only way to correct
that is to go back in and do surgery and surgically remove
that scar tissue and replace the implant. The aspen after surgery
gives us a non operative measure to treat that and what is
involved is this. The impetus for that contracture hardening
seems to be bacteria and it can be seated from the breast
because iÄôs an open gland into the environment. The
bacteria thatÄôs found on that capsule is normal bacteria found
on the skin but what happens is when it lands on that implant it
acts differently than bacteria elsewhere in the body and forms
whaÄôs called a biofilm like a shell around it so the
antibiotics canÄôt eradicate it. What the aspen after surgery
treatment does is an ultrasound device specially designed for
the breast that imparts energy to break up that biofilm so that
the antibiotics prescribed will eradicate the infection and
soften up the breast implant again. When we see patients the
success rate depends on how severe the contracture is and
how long theyÄôve had it. But in the typical patient that
comes to us if it hasÄôt been too long, if it isnÄôt too far
advanced but itÄôs distorted enough that they can see the
distortion and feel the hardness we get about an 80 percent

figure and especially after having children. Oh well, Grace,

organ in your body is your skin. Without your skin you actually
canÄôt live and there are some people unfortunately who do have
serious problems where they lose large areas of their skin and
they actually have a very tough time surviving. How do you take
care of your skin is a good question. It sort of depends
upon where you are in your life for example. Obviously if
youÄôre a little baby and you canÄôt take care of your own
skin, you need your parents to make sure that they change your
diaper frequently and use moisturizers and keep you from
getting an infection or an irritation. As you get older in
your teenage years you need to make sure that you take care of
your skin that may be too oily and whatnot, you may be prone to
acne so you need to wash your skin and use appropriate acne
medications and things like that if necessary. As you get older
you need to make sure that your skin doesÄôt get too dry so
you donÄôt want to bathe too many times with harsh soaps and
waters and your skin can get dry and then develop rashes. And
then all throughout your life you want to make sure that you
donÄôt get too much sun exposure that can lead to skin
cancer and melanoma. As our skin gets older especially if weÄôve
had a lot of sun exposure our skin loses the elasticity of the
skin, it becomes instead of when youÄôre a little young person,
if you pull on somebodyÄôs skin it kind of bounces right back
and goes back to normal, as you get older if you do that it
takes a little bit longer for it to go back and as time goes on
you start getting wrinkles and your skin loses a lot of that
sort of tensile strength that it once had. A lot of that is
normal aging but if you go out into the sun itÄôs much more
accelerated. The scalp can be the site of a lot of conditions
also and if youÄôve never had any disease in your scalp you
might not think about it. One of the most important ones is
whaÄôs called alopecia. If youÄôre losing your hair
especially if youÄôre a woman that has got a lot of hair even
if yoÄôre a male that yoÄôre going bald I mean thaÄôs
really distressing to a lot of people. In addition to that
there are a lot of inflammatory diseases that can affect the
scalp, falliculitis, psoriasis, seborrheic dermatitis. And then
thereÄôs really sort of terrible diseases that can
sometimes get into the scalp. ThereÄôs a thing called
dissecting cellulitis which can cause really large boggy areas
of inflammation and pus can drain out of the scalp. IÄôs
almost like severe acne but it just affects the scalp as well.
So if you do have something up there, thatÄôs probably a good
idea to get checked out by a dermatologist and they can make
a diagnosis for you and then it can be treated appropriately. We
are excited to welcome in the studio Dr. Preeti Malladi, she
is one of the top minimally invasive bariatric and general
surgeons in the entire Dallas, Fort Worth area. Dr., welcome to
the program. And leÄôs get right into it, whÄôs the
perfect candidate for bariatric surgery? Well, obesity is a
spectrum and some patients need to lose a lot of weight and some
patients need to lose just a few pounds and bariatric surgery is
meant for the patient who needs to lose a lot of weight. So we
measure this by body mass index which is a combination of your
weight and height, itÄôs a formula. And we look at patients
who have a body mass index over 40 or a body mass index over 35
with other obesity related medical conditions like
diabetes, high blood pressure, heart disease. So thaÄôs one
way that we look at candidacy for surgery. Very nice, now I
hear that insurance, possibly insurance can help out, is that
the case? ThaÄôs absolutely the case. The only thing that
people need to understand is that bariatric coverage is
almost like having dental coverage, you either have it or
you donÄôt. ItÄôs not under general coverage and so if a
patient doesnÄôt have coverage then we can work with them with
financial planning, but if they have coverage then there are
certain criteria that the insurance companies require you
to meet. But in our practice we help them understand if they
have coverage and also to go through all the requirements to
help them get approved. There are different types of bariatric
surgeries. Can you explain what is best? How would each
individual know what is best for them? Well the most common
bariatric surgeries are the laparoscopic adjustable gastric
band, the sleeve gastrectomy and the Roux-En-Y gastric bypass.
And those three operations have different plusses and minuses. I
really encourage a patient to talk to their doctor because
iÄôs really based on the patientÄôs medical and surgical
history and some other medical conditions that they may have
because each operation affects medical conditions differently.
And so I would say it needs to be a conversation with the
physician involved. Dr. Malladi, you are a minimally invasive
surgeon, so what does that mean? I actually did a fellowship in
minimally invasive surgery because I wanted to make sure I
could get the best outcomes for my patients and traditional
surgery requires a large incision. To see the rest of

supplements. Some people say it makes them feel better but
iÄôs really not scientifically proven. From a scientifically
proven situation, herbs and supplements juts doÄôt do that
much good. Now they do help some people feel better. That may be
placebo effect, iÄôs hard to know. It also may be something
we just scientifically doÄôt know yet. So if it makes you
feel better, well by all means, take it, itÄôs not going to
hurt you unless youÄôre doing mega doses. IÄôs better to get
from a scientific standpoint your herbs and supplements in
your foods and fruits and vegetables. Anything green,
yellow, red or orange is going to be good for you and have good
herbs and supplements in it. Now, the kinds of things that
are bad for you and they have been proven to be bad for
example is milk thistle, thaÄôs a no, bitter orange,
thaÄôs a no, ginkgo biloba that people use to help their
mental function, thatÄôs not going to hurt you but it just
wonÄôt work. And especially stay away from the expensive
brand name stuff, they just go right through your system and
all youÄôre treating is the sewage system. For additional

which starts in the surface and involves subsurface structures
including skin, fat, muscle and bone. So the way we determine
the best treatment for anybodyÄôs aging face is by
looking at the extent of aging they have in each one of those
layers. The CO2 laser will address your skin surface
problems like texture and fine lines and deep lines and some

but after examining her I determined that CO2 fractional
laser was probably the best choice for her. And she had the
added advantage of having tremendously great response to
the CO2 laser by improving her elasticity. My only knowledge of
cosmetic procedures was filler and botox and I was told that I
would probably need a facelift a few years ago by someone else
and I was dreading the thought of going under the knife and
that wasnÄôt for me so I was very thrilled when she told me
this is basically a facelift and sure enough I had it 5 days ago
and I am absolutely thrilled with the results and whatÄôs
even more exciting, she says the results are even better as you
go a few months down the road so IÄôll continue to see
improvement. I am completely blown away that for as simple of
a procedure that it is, it has completely changed my life. The
response of each individual patient to these treatments is
very, very different. It depends on their genetics, on their
lifestyle and so many other things, the amount of stress
they may have at that time. So you cannot really have a
cookbook that every patient is going to need the CO2 radio
frequency and dermal fillers even if they look the same at
the first visit. So you really have to step back and look at
their results and see how the results show up over the next

of always wanted to have larger *** but I was married and my
husband didÄôt want me to do it and had a bunch of kids and
so I had a life change in the 9Äôs. One of my good friends

Afterwards I was here for hours recovering and I had a friend
take me home and I slept through the night. Waking up I had very
little pain. After about 4 or 5 days I was pretty comfortable in
going and doing. I took a couple of days off of work and then I
was back at work. When Kimberly came in she needed some changes
in her life. She needed her *** done or wanted them done
and she wanted a new figure so thaÄôs what we did. ItÄôs
great, I love it, he did a great job and IÄôm very happy, very
happy with it. Her *** are fabulous if I do say so myself
which I will. They fit her body. She told me a minute ago she
looks like she does now when she was 28 years old and thatÄôs
what we did for Kimberly. We just gave her a boost of self
esteem, sheÄôs happy. ItÄôs win, win all the way around. Dr.
Barnett was very personable, very funny and nice and he takes
time to know you and he lets you know him and so then you kind of
have a relationship before the surgery, so you feel very

doctors and what I do is I receive referrals from other
doctors and then the doctors, they see the patients and if
they tell me that they need to be referred to having a CT scan
