Intake Coordinator--So do you know who your TRICARE primary care manager is?
Frank Levine--Yes...Dr. Hodd. Intake Coordinator--Is Dr. Hodd a TRICARE
network provider? Frank Levine--Yes, he is.
[Pause] Can you give me some information about
my TRICARE costs? Intake Coordinator--As I mentioned before,
in regards to your Medicare coverage, we are not able to bill Medicare for services
provided. We will bill TRICARE for Life and they will reimburse us as the secondary
payer. Keep in mind that your costs could be significant. For example, for this
outpatient visit, you may be responsible for up to 80% of the billed charges.
[Pause] For more information, you can contact
your TRICARE for Life representative (WPS) and I'll get you that phone number.
Frank Levine--Thank you. I'd like to know what I'm getting into financially here.
Intake Coordinator--Now I'd like to ask if you have any medical information from
a different location or provider that your physician here may need to help you
in your treatment. Frank Levine--Well, I did see a Dr. Jones
last year when the pain in my shoulder first started. He prescribed some medication,
but it didn't help very much. Intake Coordinator--We ask that you will
complete a release of medical/health infor- mation form to allow any of your past medical
information to be shared with our medical facility.
Frank Levine--That's fine. Do you have the form that I need?
Intake Coordinator--I'm sorry that we are not one of the approved VA pharmacy site
for the VA TRICARE Pharmacy program. If this were an approved site you could fill
any prescriptions from today's visit right here; but since we are not an approved site
your prescription would have to be filled at a TRICARE network pharmacy.
[Pause] It looks like you are not registered, so I
will have to enter some information into our system so that we may schedule you for an
appointment. Frank Levine--Great. Thank you.
