The Tapestry project care team model brings together a variety of health care professions.
Pharmacists, nurses, social workers and others review the TAP report. That's the report summary
the volunteers created after their visit to the client's home. Together they try to work
out how best to address the client’s needs and goals as outlined by the clients themselves.
They look at the report proactively and often re-connect with the client with the goal of
trying to get ahead of crisis issues. They also try to examine the report for clues of
how best to match the client up with the community resources and programs they need so they can
successfully age in Hamilton or other communities. In every case, they are led by the clients'
self identified goals and by the newly acquired knowledge picked up on the volunteer visits.
