one of the dilemmas has been when should those patients receive post-mastectomy radiation?
The two characteristics of the primary tumor that would almost always mandate post-mastectomy
radiation would be a primary cancer 5 cm or greater or a patient with four or more positive
lymph nodes. In those two situations it is clear that the post-mastectomy radiation decreases
the risk of local recurrence and also provides a better 4% survival benefit.
There are also now research trials looking at whether post-mastectomy radiation is helpful
in patients with one, two, or three positive lymph nodes and the results of those trials
should be available in the next two to three years.
