Now you know what your ideal career is it is time to start applying for jobs.
You can do this in a number of ways. You can go to employers directly

One of the things that people ask me is when should they disclose their disability.
Should it be before the interview or during the interview.
My personal preference is to only disclose I have a disability when invited to come along to an interview.
I don’t actually put it as part of my application.
Its very much a personal preference and up to you when you feel most comfortable disclosing your disability.
When you do go to interview, go along with some well prepared examples of experience
and achievements you have got in the workplace.
Voluntary work, or if you don’t have any work experience consider other parts of your life.
It could be employing carers where you have some experience or you’ve done some work experience
as part of university, school or college. A really good formula you can have in the back of your mind is STAR.

and what you did in that situation; what was the task you did?
What was the outcome and what was the result? Good luck with the interviews.
