IÄôm Grace Sells and welcome back for another edition of Best
Docs Network, the show thatÄôs featuring some of the best
doctors in the Houston area that are really changing peoplÄôs
lives. Up first today on our show weÄôre going to see how
KimberlÄôs life was changed by podiatrist Dr. Gabriel Maislos.
A bunion deformity is a deviation of the first
metatarsal and the second metatarsal so basically you know
what people see of their big toe is this. So the big toe is
pointing this way and this bone is going out this way. When
iÄôs not matching up well you get bone growth. And so bone
starts to build up right here and it also starts to build up
top. I spend about 85% of my time traveling so IÄôm in and
out of the airports. As a training manager I do a lot of
facilitation so IÄôm on my feet a lot walking and it got to the
point where even flat shoes what most people would think is
comfortable shoes became painful. Her surgery was an
Austin Bunionectomy but it was a little bit different because on
the x-ray I discovered that she had severely arthritic joint.
She had fracture fragments. So these fracture fragments never
healed and because they were constantly moving it was severe
pain. It caused an enormous amount of bone growth on the top
and on the side. So I was really excited to do her surgery
because I knew it would provide immediate relief. IÄôll be
wearing a boot for at least another 4 weeks and then Äôll
come back for another reassessment and then hÄôll
determine then whether I can take the boot off and then just
start walking in normal shoes. People always ask you know why
do I have this problem and the biggest component, genetics. So,
if you look at your parenÄôs feet or your grandparenÄôs
feet, iÄôs basically telling you that if you see a bunion
forming that chances are your feet are going to develop these
structural deformities. And then obviously for the females,
iÄôs wearing the high heels. You know, thereÄôs a price to
pay for high fashion. You wear the high heel it puts your foot
in a position which predisposes you to a bunion forming. I would
love to be able to get back out there and be pain free. A lot of
times, I couldnÄôt wait for the day to end because I would be in
so much pain. So Äôm looking forward to being able to be on
my feet for longer period of times pain free. The look on the
patients face when they come in and they see the results and
theÄôre happy with the results and they see that instead of
having a crooked toe that iÄôs actually straight and that the
pain that they had before is gone so thaÄôs the rewarding
part to me. Yeah, I lost a couple and the rest of them
started getting loose and it was something that took about five
or six years to happen. It just suddenly came on though. Yeah,
George, his teeth were in an end stage in that they were decayed
and he also had periodontal disease, bone loss around his
teeth. His teeth were chronically infected, they had
become loose. Also, there was a cosmetic problem with his teeth
and George is a very outgoing person and likes to smile and
has a great smile and so his smile just didnÄôt suit his
character not to mention the health of his teeth. It was
really driving him down. I was kind of getting to the point
where things that required real chewing particularly in the back
of my mouth was getting more and more difficult. With George what
we did was removed his teeth. Now in him we focused on his
lower jaw and from a financial standpoint many individuals
decide to maybe focus in one jaw versus the other. Now granted
with him we could have done both of them simultaneously but for
whatever reason and oftentimes iÄôs schedules, their busy
work schedule, also finances that theyÄôll sometimes stagger
the case to suit them better and with George we focused on his
lower jaw and we removed all of his teeth upper and lower but we
put the implants immediately in his lower jaw. Äôm feeling
fine now. IÄôm back to the point of eating anything,
thaÄôs no problem. WÄôll continue on. Äôm not quite
through with all the procedure but just about. And that is a
misconception that weÄôre seeing right now with the
advertising where it talks about get this all done, completed in
one day. Reality is the final is usually done later down the road
once the body has fused to the implants and the textbook answer
for that is at least three months. But it can be as long as
anywhere from three to six months but standard of care is
to keep the patient in a provisional, a prototype if you
will, for a period of at least three months. According to my
experience no question about it, donÄôt be waiting on it. The
earlier you start some treatment, clearly the better.
Start sprinkling your oatmeal with some cinnamon. Studies show
that this antioxidant rich spice may reduce blood pressure and
lower stress. This is such a fabulous event, iÄôs called Go

about how IÄôm dealing with heart disease and how to have an
optimistic and positive attitude while going through something or
anything difficult in your life. I keep laughing, I keep smiling,
try to stay positive about things. I could be stuck in the
hospital but instead Äôm home with my family. The biggest way
to prevent these problems is education. So the main reason
for this type of event is to educate and through that we
raise funds and can get the word out. Number one, it educates
people on what has happened over the last several years and it
brings to the forefront with women in particular with Go Red
for Women to listen to your body and know your symptoms. The most
important thing about cardiovascular disease is iÄôs
the number one killer of women in this country and 50 percent
of women or men with cardiovascular disease die
suddenly without any symptoms, without any warning. I just
believe getting the word out to women especially, we always are
tired and rundown and raising kids and going here and there
and we might just think oh wÄôre just tired, my neck
hurts for some reason and iÄôs no big deal but it can be a sign
of a heart attack or a stroke. Women you know and theyÄôre as
we know as men are different and they present differently.
Fortunately stroke type symptoms are about the same but again
another large focus for us is heart disease and they are
completely different in the way they present as a general rule
than men. Cardiovascular disease and the awareness for women is
such a tremendous topic and thaÄôs why Go Red for Women is
such a marvelous cause and I just want everybody to support

happen for several reasons. One was they just came loose and
they can come loose just because they were never put in correctly
or sometimes they just come loose. And the second reason is
infection. About two and a half percent of the hips put in
nationwide get infected and once theÄôre infected they have to
come out and you have to start over. Occasionally I see like
this patient we did recently had been infected for several years
and had already replaced, revised her other hip that was
infected but shÄôs in bad health and she was in a nursing
home for many years and had not walked until we revised the
first hip. She had not walked in two years and then after
revising the first one she actually got out of bed and was
able to ambulate her in the house and for short distances.
But the second hip which had been chronically infected
essentially put her back in the bed and was bedridden. And so we
just replaced hers recently and in this case her femur was
broken in several places, infection was really bad. It was
a very complex 7 hour surgery, but it looks good. Twenty years
ago the mortality rate on a hip fracture was around 40 percent
of the people died within the first year. And currently that
number is around 10 to 15 percent and the difference is
fixing it and replacing it more often than trying to fix it,
replace it with a new hip and get out of bed and get back,
just get back to activity as soon as we can. I think that hip
replacements for the most part have just been one of the few
miracles of modern science. I mean their, you can take someone
who either has arthritis or a broken hip that literally cannot
function in life, the pain is so severe. And then do a typical
normal hip replacement and iÄôs an hour surgery and you
can have a hip thatÄôs absolutely normal. In fact
ninety five percent of the hips get so good that people forget
they had one and it becomes a normal hip and they can go on
with their life and do everything in their life that
they wanted to do with extremely few restrictions once theÄôre
healed up. I really love what I do but what I love most is
trying to take somebody whose life has really just totally
destroyed because of whatÄôs wrong with them because of pain
or disability and trying to get their life back and that
encompasses much more than just the surgery and thaÄôs what I
enjoy. ItÄôs getting someoneÄôs life back. Experts
say weight training should be done first because itÄôs a
higher intensity exercise compared to cardio. Your body is
better able to handle weight training early on in the workout
because youÄôre fresh and you have the energy you need to work
out. How much sleep do you need? That varies from person to
person. In different times in your life you may need different
amounts of sleep. So how much do most people need? Well, some
people need 3 or 4 hours, those are the rare people. Some people
need 10 or 12 hours, those are rare also. Most people need 7 to
8 hours to get a healthy nightÄôs sleep. Now during
sleep you go through cycles. About every 90 minutes you get
back into light sleep and then go into deep sleep. So about
every 90 minutes you are in a lighter level of sleep and
thaÄôs why at about the 90 minute timeframe or a multiple
of 90 minutes is when youÄôll wake up to use the bathroom at
night or wake up to go get a sandwich or something. So at any
rate get 7 to 8 hours of sleep a night at least. And it is true,
sleep when itÄôs dark, donÄôt sleep when itÄôs light. Early
to bed, early to rise makes a man healthy, wealthy and wise.

home page. For numerous years IÄôve had varicose veins. Äôm
a registered nurse and so I have been on my feet for twenty years
and also heredity has played into that and so just a lot of
varicosities and some pain, aching if IÄôve been standing
on my feet for a long period of time and then just
unsightliness. Varicose veins are a symptom. TheyÄôre a sign
of an underlying disease or abnormality in the venous
circulation. In your legs there is no heart to pump the used
blood back up so the pumps are your muscles and when the veins
become too large and one way check valves have failed now all
the pumping doesnÄôt work and the blood pools in the legs, it
creates high pressure which results in bulging, enlarged
veins that we call varicose veins. My dermatologist
recommended Dr. Morgan and he was very forthcoming with
information. It wasÄôt a lot of extraneous things that I
didÄôt understand, it was very to the point you know exactly
what he could do for me and what the benefits, what the side
effects and it gave me an opportunity to just weigh the
pros and cons of whether I wanted to go forward. The
treatment is to close all the veins that are running
backwards. Any vein that is not pumping up out of the leg is
only hurting, it doesÄôt help and so by removing or closing
those veins you restore the circulation out of the legs back
to normal. The procedure itself probably took 30 to 45 minutes.
I was able to get up off the procedure table right afterwards
and walk. I did have improvement and cosmetically which is a
wonderful side benefit but I also have decreased swelling,
decreased aching, really itÄôs very minimal. She got a great
result. Her swelling has gone away completely, her pain has
gone away completely and her varicose veins have gone away

thickening of their heart muscle. TheÄôre already not

When I originally started to even look into this after I had
talked to my friend, I was probably between 335 and 340. I
had been that for 10 or 15 years, been over 300 pounds for
at least 15 or 20 years. WeÄôre giving them back some of that
control in that one area where theÄôve had a problem. But
iÄôs a dangerous area. You canÄôt walk around at 310
pounds and think everythingÄôs going to be okay for very long,
iÄôs not. TherÄôs been times when Äôd lost weight before.
IÄôve gotten down to 225 before but 6 months later Äôm back at
300 and that was always my issue. And after you do that 2
or 3 times you know why do you want to do it again because you
know whatÄôs going to happen. I actually had a buddy of mine
that did the surgery probably a year before I started looking
into it and he had had great results. His weight loss was
incredible from what Äôd seen and IÄôd known the guy for
several years, so and he was completely, completely happy and
excited with everything, so thaÄôs when I started looking
into the surgery myself. Buck has lost 115 pounds in about 6
months after having a sleeve gastric. I mean thaÄôs a good,
good outcome so far. WeÄôre only partway through, wÄôre
going to expect more weight loss beyond that. Although hÄôs
clearly getting down towards his ideal body weight which is what
wÄôd ideally like to happen. I started off at 335 or 340
January of 2011 and iÄôs what February of 2012 and Äôm at
186. ItÄôs like being reborn. Before I guess I just kind of
just muddled through life, no excitement, just do what I had
to do to get by, no energy, no fun. Now, I mean excitement,
fun, romance, social life, things that I didÄôt have
before. He changed my life. IÄôs a whole new world every

morning, the same day I was out and back at home. It was a
really good experience. It really made a huge difference in
her confidence and self esteem. It may seem like a small thing
to someone who didnÄôt know her or just a casual observer but in
her mind it made a world of difference. It just really
changed her personality. It kind of brought her out of her shell,
she just felt better in a bathing suit and photographs,
just being out socially so it was a real blessing for her over

life was changed when he was given a brand new smile by
cosmetic dentist and smile specialist Dr. Guy Lewis. I had,
the 4 front teeth were very crooked. Two were going forward,
two were going backward. I found I had to wear braces for quite a
few years and that just wasÄôt a good alternative for me so I

primary care physician because my blood pressure was a little
elevated and my EKG reading was a little off. She suggested that
I take a stress test and have some other procedures done just
to determine the causes of that and it was determined that I did
have blockage in one of my arteries and a stent had to be
implanted and then reality sunk and Dr. Varughese talked to me
during the recovery time and I asked her in tears I asked her
what was going on? I asked her was I about to have a heart
attack? And she said yes. Yolanda Garcia to this day,
sheÄôs so thankful that her primary care was cognizant
enough to send her for evaluation because many times
primary doctors or nurse practitioners who see all of
these patients they may not recognize because the
patientÄôs not complaining much so I think this is a wakeup call
for all of us as physicians and healthcare providers to really
focus on prevention, evaluating the patient early, getting them
to the right specialist on time. Because of her, IÄôm able to
take care of my clients, because of her I have a better life so I
really am appreciative of all she has done for me. Well
coronary disease in women especially is the number one
killer of women in this country and pretty soon will be the
number one killer of women all over the world. And in Hispanic
women especially there is a high prevalence of diabetes so
therefore diabetes lends towards cardiovascular disease and
coronary disease especially. IÄôs just so important to
educate our Hispanic population, our Indian population because
Indians die 4 times the rate of any other population from
diabetes and cardiovascular disease. People are hungry for
knowledge. ItÄôs just for the lack of knowledge we perish. The
symptoms is I had headaches, a lot of congestion and infections
in the throat, a lot of mucous in the back, ear aches, so much
stuff going on. Jenice Martinez is a patient who came to my
office complaining of nasal obstruction, facial pressure,
constant headache. She has been treated under multiple
medications including antibiotics, antihistamine,
nasal spray without really, without any good success. I went
on but then I started getting infections and I was on
antibiotics for four months in a row that they kept giving me. So
I was like this is not good. So when we examined Ms. Jenice
Martinez we found that her natural sinus drainage pathway
is obstructed by chronic inflammation, chronic infection
so we offered her the option of doing balloon sinuplasty. The
best part was the breathing. I could breathe again. The
headaches little by little started going away. Now when I
sleep I would breathe real loud. My husband would always say that
and I donÄôt do that no more. So when you have a chronic sinus
disease that passage is obstructed, iÄôs narrowed by
chronic inflammation, chronic infection. So what we do is we
put a small balloon in the passage and we open up, inflate
the balloon and open up the natural passage preserving the
normal anatomy. And I donÄôt sound like I have colds all the
time. Before I was like I was sick all the time. And now Äôm
doing great. I can go out, do everything and I feel good. One
of the advantages is the quick recovery. ThereÄôs really no
down time to balloon sinuplasty. The procedure can be done in the
office under local anesthesia or topical anesthesia with some
nasal spray. TherÄôs not really a recovery time for
balloon patient. Can you know go back to the daily activity the
next day, and compared to traditional surgery which
