What do I do here at MCD?
I manage the Telecommunication Program (TEP)
What is TEP?
It is a telephone communication equipment program
The state of Maine, Division of Deafness
helps donate money to our TEP program
How do you qualify for this program?
1. you have to live in the state of Maine
2. you need to have either, visual loss, hearing loss (Deaf, Hard of Hearing or Late-Deafened) or any other kind of disability.
3. also need to have low-income.
with those 3 things you would qualify for the Lending Program.
The lending program is only for amplified phones, tty's, light flashers.. anything related to phones is included in the program.
To apply for the lending program we require that you fill out the application form.. make sure your name, address, birthdate-very important, and signature is on the application.
second, we would need along with the application is proof of your income.
either a bank statement, social security statement, pay stub or tax return from the most recent year is needed.
and third we need proof of disility
for example: an audiologist signature proving that you do have hearing loss ...
or a doctor's signature
or if you have a diploma from a deaf school.
one of those three choices should be fine
again, to apply for the lending program we would need: the completed application form,
along with the proof of disability and proof of income.
Once we receive it all we will review it and let you know if you qualify
what happens when you qualify?
MCD decides based on your application, what phone would be the best match for you
we will pack it up and ship it through UPS to your home.
Understand that it is LENDING program, meaning you are borrowing the equipment..
If you end up not wanting the phone anymore or move out of state
can you give it to a friend? NO!
you have to send the equipment back to us here at MCD
If the equipment itself is broken, or not working right
let MCD know and we will replace the equipment for a new one
If you need to replace your equipment we will need your most recent and up to date income information in order to send you your replacement
If we review your application and you end up not qualifying for the lending program
you could qualify for the Cost-Share program
whats the difference between the Lending and Cost-Share program?
MCD and the client would pay for the equipment, so it would be a reduced cost for the client.
how much you pay for the equipment depends on your income
The client can pay with cash, check or money order
once we receive payment we will box it up and ship it to your home
with the cost-share program you can keep the equipment.
If the equipment breaks understand that because you bought it you would have to pay for a new one under cost-share program
