Add Requests To access SAM go the office of accessibility website at www.utoledo.edu/utlc/accessibility.
Choose the second button down on the left hand menu. It should say Student Accessibility Management, SAM. The computer will be
redirected to a new page with an introduction and updates to SAM. To access SAM click on the SAM banner located at the top of the page,
this will hyperlink you to SAM.
In the middle of the page there is a box with sign in information. Use your utad information to log in. First enter in your username,
and then enter in your password, choose sign in. The very first thing you will see is important messages that will tell you what still needs to be
done for accommodations. Under the 3 step process to request accommodations, in step one choose the appropriate term.
Then in step two enter in the course registration numbers or the crns for which you wish to receive accommodations for.
Click on step 3 to continue to customize your accommodations. Only the accommodations that you qualify for will be seen on the screen.
For each course choose the accommodations in which you wish to use. So if you only need e- text for the first course only choose e-text.
If you need e-text and braille for the second course choose e-text and braille services. When you are finished click the submit your
accommodation request. The status will show as processing until your accommodation is approved by your specialist.
Once your accommodations are approved the status will show as approved. You will receive a copy of the advocacy memo when it is sent to
each of your instructors. Please discuss your accommodations with your instructors, and contact OA if you have any questions.
