Services whenever you have a question about the plans being offered to you or your deadline
to enroll.
Your deadline to enroll in the VSP vision insurance is no later than 30 days from the
first day of your appointment with the University. We must have your completed forms by that
date.
Your coverage will then begin the first of the month following the timely receipt of
your application.
The monthly premium for VSP vision depends upon who in your family you are covering.
VSP vision provides certain dollar amounts or percentages of coverage depending upon
the services being provided. Frames are covered every other calendar year and spectacle lenses
or contact lenses every calendar year.
You may search for in-network vision providers on the VSP website: www.vsp.com.
Please visit our website to review the full VSP plan details. You may also wish to compare
VSP's coverage to the supplemental vision coverage offered through EPIC Benefits+. You
may elect none, one or both the VSP and the EPIC Benefits+ plans as a new employee.
We encourage you to use our Benefits Walkthrough as a tool to help determine what your monthly
paychecks will look like after your elected insurance benefit premiums have been deducted.
This Walkthrough does not enroll you in any coverage, so you may click on as many plans
as you'd like in addition to or instead of the vision insurance, just to see how different
enrollments affect your net take-home pay.
If enrolling in VSP, please complete an application and hand your completed form to your department
supervisor, HR contact, or into UW-Madison Benefits Services. Do not leave your completed
application form on anyone's chair or desk, but instead physically hand it to someone
so they may sign it in as received timely. Your 30-day deadline applies to the signed-in
date. Ask for a copy of your signed-in form for your own personal records.
If you miss your initial 30 day enrollment deadline, you may have limited future enrollment
opportunities.
New enrollments may be allowed due to certain life events; completed forms would have a
deadline of no later than 30 days from the event. Please contact UW-Madison Benefits
Services if you experience a life event.
