Hi again everyone, Äôm Kandace Krueger along with Jim Knox and
wÄôre back again with another edition of the Best Docs Network
featuring Forest Park Medical Center Dallas and Frisco.
Of course, Jim, Forest Park Medical Center is not just one

Exactly right, and Forest Park Medical Center has some of the

Center, like our first doctor, spine surgeon Dr. Rick Burg.
Back pain just sort of snuck up on me after I had my son.
It gradually got worse to the point that I needed somebody to
help me out.
And that led me to injections, therapy, things of that nature
and just didnÄôt work for me.
Finally somebody suggested that I see Dr. Burg and so I made an
appointment with him.
Well the first thing we need to do when we see patients come in
is get a proper diagnosis.
We need to get the proper imaging studies, we need to
understand the problems wÄôre treating as spine professionals
and once you have that understanding you come up with
the proper diagnosis, you can come up with the proper
treatment.
So most people get a long regimen of conservative
treatments including all the therapies, medications, pain
management treatments with injections, sometimes spinal
cord stimulators, there are many different technologies out there
that we have to try and actually have to consider.
The last thing you want to do is make a patient worse because you
picked the wrong treatment for them.
Dr. Burg looked at my x-ray.
Right away he could see that I had spondylolisthesis.
The condition I had wasÄôt going to get better.
Spondylolisthesis needs to be fixed surgically, thereÄôs
just, you know you can rehab that particular condition if you
havenÄôt declined to the point that I had already, so really
therapy wasÄôt going to get me strong enough to deal with that.
Eventually it led to needing you know a pretty invasive surgery.
And what I saw on the x-rays was something we commonly treat that
has a very good likelihood of significant improvement of being
treated.
When you see spinal instability causing pinched nerves leading
to leg pain, numbness, tingling and weakness and a lot of back
pain thatÄôs mechanical in nature, thaÄôs something that
can be fixed with surgery.
After an extended recovery period, after some recovery
these patients do very well.
Honestly I have healed so quickly.
IÄôs just over six months for me and Äôm almost completely
fused so I mean I feel fantastic.
IÄôs like the pain I had is gone.
Radiology Departments are very commonly in hospitals.
The difference here is that this is primarily a surgical hospital
and our primary purpose is to support our O.R.
and we have 14 O.R.
suites upstairs.
We have both pre and post operative imaging.
Here at Forest Park I work more closely with the staff members,
kind of the segue between the director and the technologist.
IÄôm here with them daily supervising what rooms theyÄôre
in, what exams need to be done, what tech goes to what
assignment.
WÄôre a full service out patient facility.
We have an MRI here thaÄôs called a short bore.
What that means is itÄôs not as coffin like for patients, iÄôs
more open.
We also have a CT scanner here, iÄôs a 64 slice that is
capable of doing cardiac as well as any other CT imaging that you
might need and itÄôs also an interventional radiology CT
scanner meaning we can do procedures that normally would
have had to been done in another room and then brought to CT for
scanning.
We can do both on the scanner at the same time.
We have an onsite radiologist from RCA that read all our
imaging.
We have an onsite ultrasound unit fully capable of doing
general ultrasound and vascular and I also have a staff member
that can do both.
Here at Forest Park Medical Center we work closely with some
of the best surgeons in the area, theÄôre definitely top
notch.
We do whaÄôs best for them and we make sure that we have all of
the equipment necessary, all of the reps that they need to make
sure that they get the job done well.
Our surgery suite encompasses four C-Arms that can be used in
a variety of times during the day and our primary O.R.
tech who happens to be our lead tech here at this facility is
Stefanie Woodlee and she has not only taken the bull by the horns
here at this facility but sheÄôs also trained each of the
other staff members that work in O.R.
and has done a phenomenal job in that capacity.
If weÄôre doing spinal fusions, that screw placement is very
important and we need to make sure that those screws go in
exactly where they need to be and imaging has everything to do
with that.
If our image is wrong then those screws could be placed
incorrectly so we work very closely with the surgeons to
make sure that we have the right image that they need to make
sure that they can place the screws correctly in the spine
where they need to be.
WÄôre actually imaging during the surgery and assisting the
surgeon to make sure that hÄôs guiding his tools into the right
proper anatomy to make sure that hÄôs doing it where he needs
to be.
I really firmly believe that we have state of the art equipment
here and Äôve got a top notch staff that can really meet any
of the referring physicians in the communities needs.

Now to our next best doctor, it is Dr. Glenn Ihde.
Once weÄôve diagnosed reflux and wÄôre sure that weÄôre
dealing with that entity then we can talk about treatment.
And a lot of people think that we just skip straight to
medications but in fact most people initially recommend
lifestyle changes.
We tell people maybe elevate the head of your bed to help gravity
take effect with the symptoms that may be occurring at night.
Avoid eating late at night, eat smaller meals, avoid carbonated
beverages and avoid some of those trigger foods that may
relax that lower esophageal sphincter and increase the
amount of reflux that can occur.
If those doÄôt work many people will go to over the
counter medications, they may see their primary care doctor or
gastroenterologist and they may prescribe acid reducing
medications.
As people get reflux more often they start using medications
that actually prevent the production of acid so the
stomach content is less acidic so when they do reflux it
doesnÄôt burn.
If yoÄôve been on those medications for more than 8
weeks or you have to take them every day to prevent symptoms or
even if youÄôre having symptoms while youÄôre taking those
medications, then iÄôs time to think about surgery.
Reflux disease is normally a problem with the anatomy at the
junction of the stomach and the esophagus and thereÄôs usually
a hiatal hernia that can accompany reflux disease in
patients and when we go and correct that anatomical defect
you can basically help restore that normal anatomy to where
patients do not have to take their medications any longer.
So the newer procedure, what we call a transoral fundoplication
tends to provide the treatment without the complications so if
youÄôre taking the medications for a long period of time, if
youÄôre symptomatic on those medications then now might be
the time to think about turning towards a surgical procedure
that can recreate the anatomy the way iÄôs supposed to work.
It is a minimally invasive procedure thaÄôs done without
incisions and iÄôs very helpful in treating the reflux
symptoms and as well as decreasing the amount of side
effects that were seen previously with the other types
of surgery.
In this community that is growing rapidly, very young
growing population, sports injuries are just everywhere.
I was at my football practice and I play quarterback.
I was throwing the football and I ended up hitting one of the
linemenÄôs facemask and my thumb just didnÄôt feel good.
IÄôve come to see Dr. Michelsen for my left wrist being broken,
tendenitis in my right elbow and I dislocated my left pinkie so
IÄôve been in here quite frequently.
With Brock and Brady I see them quite frequently.
TheÄôre great kids, theÄôre competitive.
Sometimes it has been an acute injury, broken fingers and thumb
but sometimes iÄôs an overuse situation.
We can come here for anything even thatÄôs just a worry.
Sometimes iÄôs just the emotional part where you think
you hurt it but when they tell you you didnÄôt it feels
better.
IÄôs very comforting when Dr. Michelsen talks directly to you
because you know hÄôs trying to help you.
I think iÄôs important to involve the patient in the
discussion, to try to help them understand because theyÄôre so
focused on I want to play in that game.
But in the long term I try to help them understand how the
bone heals and how the body works, the potential problems
they can have because of it and try to carry them through that.
HÄôs just like a friend that knows how your bones work, how
your muscles work and you can just come to him for
reassurance, just really comforting to know someone who
knows what to do when you doÄôt.
My thumb is 110%.
I can move it even a little more now after all the treatment and
what they told me to do, iÄôs perfect.
Sharon has a question for Dr. Troy Diehl.
What is the difference between an MRI and an x-ray?
So an x-ray we utilize in our office very commonly is to look
at bone.
It gives us a very good in depth view of the bony anatomy and
exactly the alignment of a joint or whatever body part wÄôre
looking at.
In regards to an MRI, an MRI is actually not a radiograph or not
necessarily radiation, itÄôs a magnet that utilizes the ability
to look at soft tissue and give us an idea of the extent of a
soft tissue injury or not.
It allows us to look not only at just tendons and ligaments but
also cartilage and the soft components that make up all our
joints and tissues.
We utilize both tests because they both function very
importantly in our diagnosis because we get a very good idea
of the overall anatomy from an x-ray but the MRI allows us to
dig deeper into the soft tissue knowing that their regular
anatomy is normal which we gleaned from the x-ray but now
we can utilize the MRI to further look into and see
exactly whaÄôs going on with all the structures that support
those bones.
IÄôs very important for Forest Park Frisco to get out in front
of the Frisco community and let them know what weÄôre about,
that weÄôre an amazing hospital that focuses on excellent
service and excellent care and that we want to be involved in
the community and really make a difference here.
This is our trick or treat on the square annual event.
We partner with the Frisco Square community down the street
and we do a huge community event for all the kids to come out and
have a good time in a safe environment and give them
opportunity to see what Forest Park is all about and what the
Frisco community is.
Today wÄôre doing carnival games, we have a photo booth for
the kids to get in and do photo booth, photo pictures in their
costumes or in any other kind of dress up stuff that is over
there, hats and glasses.
We also have some fun treats with cupcakes and pecan pies and
pumpkin pies as well as traditional Halloween candy as
well.
So a huge part of making Forest Park the destination for better
health is to make sure that wÄôre providing our patients
with the highest level of care possible not just from a quality
of care standpoint and making sure that their outcomes are
positive but also from a true care standpoint making sure that
our patients actually leave the facility feeling like they had
an experience here that they wouldÄôt have gotten at any
other facility.
They really feel cared for, they connect with their caregivers,
their nurses and the techs that care for them both in surgery,
preop, PACU, on the floor and in patient or if they have a
patient to go to our ICU they find amazingly caring nurses
that really make a difference with their patients.
WÄôre a very enthusiastic group both the directors but
also the staff and we really set the culture for the hospital as
a whole to bring people together.
We have lots of volunteers out here today that are talking to
the community, talking to the parents and the kids and really
just interacting in a very special way that shows what

iÄôs bariatric surgeon Dr. Joel Rodriguez.
I got up over 300 and something pounds.
I had no energy, I just wanted to sit around.
I didÄôt want to do anything, it was affecting my job, it was
affecting my family.
I coach and my coaching was affected.
I didÄôt get out and interact with my kids.
Also, IÄôm diabetic or I was diabetic.
When Coach came in to see us he was concerned about two things.
One was his decreased energy level, two was his diabetes.
So we decided to perform a laparoscopic Roux-En Y gastric
bypass on Coach because it is probably the most effective
surgical option as far as diabetes management.
I did the typical diet stuff.
IÄôd lose 20 or 30 pounds and then I would gain it back plus
20 or 30 pounds.
I was what I consider myself a stress eater.
Under a lot of stress I was eating all the time even when I
wasÄôt hungry.
And so talking with Dr. Rodriguez we felt like the
bypass surgery was going to be the best for me.
The procedure is performed by dividing the stomach into two
parts, creating a small gastric pouch, bringing up a loop of
small intestine and attaching it to that gastric pouch so
thereÄôs a segment of the GI tract that is bypassed.
There are two components to that operation, one is restrictive.
Basically hunger is satisfied at an earlier point.
The second part of that operation is malabsorptive.
Nutrients, calories that you otherwise would absorb, youÄôre
not going to because of that malabsorptive component.
When I had surgery I was 324 pounds and Äôm down to 216,
217 and it has worked.
Coach is doing great.
Literally the day after surgery he was off his diabetes
medications.
As a physician itÄôs a really rewarding thing to see.
I feel better, I feel like I look better, my clothes fit
better.
Coming to Barker Bariatric has definitely changed my life.
Meeting Dr. Rodriguez and the procedure and the way he treated
me was a blessing to me.
Generally they care about what happens to you and your health
and IÄôm extremely happy for that.
Did you know at Forest Park Medical Center the patient ratio
of care is typically four to one, one nurse to every four
patients versus the typical five to one or six to one which is
characteristic in most hospitals.
I think what makes Forest Park unique and different is truly
the culture that we have.
A lot of places present that they have the patient care and
the employee satisfaction in their culture, but when you walk
into Forest Park you feel the difference.
Our directors over our units are very passionate about what we do
and since wÄôre a physician owned facility, the physicians
that are here are very involved in the care and in the decisions
that we make down to the in-patient level.
One thing I really love about Forest Park and I tell everyone
that I talk with in my nursing career about Forest Park, I like
to talk about the camaraderie thaÄôs here.
I really enjoy just a positive, energetic setting with the
staff.
Everyone works quite well together.
Inter department, just you know pharmacy, ER, X-ray, the
doctors, the nurses, itÄôs just a really cohesive unit and I
really enjoy that.
And I fell like every day I come here I enjoy my day and that
says a lot because nursing can be really stressful.
To be able to come here and feel the support from my teammates
and to have a good day at the end of every day, iÄôs
something I really appreciate.
The best thing about working at Forest Park is that we are a
family here.
All the directors know each other.
We know each other inside and outside of the hospital.
We know how to work well with each other and our
administration here at Forest Park really appreciates and
shows us how much they appreciate us, not in big ways
but in small ways every day that really make a difference in the

One thing that really stood out about Forest Park was just the
quality of patient care and just the way the patients are
treated, the not only high level of patient care thaÄôs
delivered but also the entire experience that they receive

The facility promotes patient satisfaction, patient care which
are all things that are very important to me and iÄôs a
facility that I feel like are vey in line with their values
and what they view.
Unless you worked at a hospital like Forest Park yoÄôre going
to find that therÄôs a lot to learn, a lot to do and a lot of
rewarding experiences to be had here.
This is whaÄôs called the Medtronic O-arm.
IÄôs a combination of traditional fluoroscopy with
also a 3 dimensional imaging component.
Within 26 seconds it will take 755 high definition images.
With the O-arm we can actually take shots of the spine in a 3
dimensional nature because the spine is a 3 dimensional
structure and the value of that to a surgeon is that they can
now see the spine in 3 dimensions and truly be able to
determine with precise accuracy their placement of the hardware.
The device also enables minimally invasive surgery by
more precisely placing the screws heÄôs going to be able
to allow the patient to have a quicker recovery time and
hopefully have a better quality of life post surgery due to the
more accurate placement of the hardware which the O-arm
enables.
We have the O-arm itself which collects the images, we have the
viewing cart which the images are reconstructed on and then we
also have the stealth navigation image guided surgery unit.
After the images are recreated on the spinal navigation unit
the surgeon is able to bring different surgical instruments
into the field that will help him more effectively place the
instrumentation into the patientÄôs back in a 3
dimensional nature.
Because this device is used interoperatively the surgeon can
use the image guided surgery unit to do the remainder of the
case without additional radiation being delivered to the
patient.
The benefit of that is to the patient that they will
potentially not need to be sent for a postoperative CT to check
the final placement of their hardware since theyÄôre able to
do that real time in the operating room.

surgeon, Dr. Troy Diehl.
I was in a lot of pain.
At that point it felt like I had a layer of shards of glass
between my kneecap and the skin on top and you can imagine
trying to bend your knee or walk with something like that.
The knee replacement entails a surgery that resurfaces the
arthritic component of a knee.
The patient that is ideal for this is somebody that has
unfortunately lost cartilage in the knee to the sense that they
now have pain, theyÄôve lost function, theÄôve lost range
of motion to where it affects their life.
My husband and I live in California.
We come here every 6 months for medical treatment so the first
thing I did was make an appointment with Dr. Diehl to
discuss my knee problems with him and he told me that I needed
double knee replacement.
Debbie was a patient of mine that came in and she had been
dealing with knee pain actually in both knees for a while.
She had definitely gone through a long course of options in
regards to treating her knee both from an arthritis
standpoint and the ability that it had on her function.
She had tried therapy and different medicines and
different injections and she just wasnÄôt happy.
When I look back it had to have been painful but it was
absolutely nothing compared to what I had before and the fact
that Äôm mobile, that I can do things again, that I feel like a
human again.
You couldÄôt talk me out of doing it again if I needed to if
I were in the same condition.
I think the idea that she comes in and is able to bend her knee
and is able to walk and do all the things that she wants to do
and enjoy her life without having to worry or to think
about her knee or let her knee tell her how to live her life, I
think is truly the success.
It is so worth it to come here for Dr. Diehl to take care of my
orthopedic needs because of the level of care and the level of

Terri has a question for Dr. Wade Barker.
Will I have excess skin after bariatric surgery?
Many patients ask about whether theÄôll have excess skin or
not after bariatric surgery.
IÄôs highly variable.
Some patients do and some patients donÄôt.
IÄôs related to age, to skin type, what kind of skin you got
from your parents and sun exposure and smoking.
Smokers donÄôt have very elastic skin and so they may
have excess skin.
Also it depends on the preoperative body shape and
obviously if you have a very large pannus to start with
iÄôs not really going to go anywhere.
But we do have board certified plastic surgeons that are in the
office with us every week.
They talk to the patients and theÄôre very eager to help
with insurance and get those things taken care of.
We usually recommend that you wait a year or a year and a half
prior to having that done but I think iÄôs important also to
have it done by a plastic surgeon who is board certified
and fully trained to do those things.
They can be fairly large procedures if thereÄôs a large
pannus left and theÄôre not something to be taken lightly.

When Mason was about 2 years old, thatÄôs when we first
noticed that oh, weÄôre starting to get strep throat
more than just once a year or so.
And things really took a turn for the worse more when Zane was
born.
When Zane turned 6 months old and Mason had just turned 3,
about once a month each child had strep throat in our house.
And then of course you can imagine my husband and myself
also had strep throat.
So strep throat can come about very quickly or very slowly.
For some people iÄôs just a mild sore throat and over the
next couple of days it develops into a more severe sore throat,
fever, headache, body aches and all around fatigue.
In other people it starts very suddenly.
Just a sore throat comes on, you get white spots on your tonsils
and on the back of your throat so at that point what we would
do is we take a swab and do a strep test which is a little
swab that goes in the back of the throat, kind of like a Q-tip
and you brush it on the tonsils or on the back of the throat and
then we put it in a little container, do a test and it
takes about 5 minutes and it tells us if the strep is
positive or negative.
With Dr. Friedman and his clinic, what theyÄôve been able
to do is treat us as a family.
So we have one child that has strep throat so we start looking
at OK who else might have symptoms?
The BiermanÄôs were getting strep chronically over and over
again.
It was mainly in their eldest son Mason.
Finally we decided after multiple therapies of
antibiotics, multiple treatments we actually tested his parents
and found out one of his parents was a carrier.
Treated the parents and he stopped getting strep for a
while, however it happened again and continued to get strep.
So eventually we had to refer him to an ENT and have his
tonsils taken out.
Now hÄôs done very well since his tonsils have been removed.
Dr. Friedman is near and dear to not only my heart but my
families.
They go above and beyond to make sure that everyone feels
comfortable, that everyone is communicated with properly, that
everyone comes in and leaves with a great experience.
