(BPMH) training for the BHPr Annual Performance Report. This lesson will also demonstrate
the data entry process using excel upload for IND-GEN and IND-FAC forms. The data for
these forms are combined in to one single Excel row in the file. The IND-GEN form, like
CE-1, uses the Excel upload process but with a slightly different structure. The IND-GEN
template contains all the IND-GEN and IND-FAC fields as one long row on one spreadsheet.
If you remember, CE-1 and CE-TTY were separated on to two different spreadsheets. We will
now demonstrate this form for you. Click the Download Template button to begin the process.
Click the Save As option to save the template to your computer. Click the Save button. The
Save As window opens, navigate to appropriate folder and save the template. Click Open button
to open the file. The IND_Template appears; review all the instructions on this sheet
before beginning your data entry. Note: It is recommended that you print a copy of the
MultiSelect Legend for easy reference. The MultiSelect Legend will provide program specific
lists for use in completing the IND-GEN forms. Make sure you review the Important Notes section.
Select the IND tab to begin your data entry work. As mentioned at the beginning of this
lesson, this is a demonstration of functionality. Certain data entry fields may be skipped during
the process of completing this form. If your program requires you to complete any of the
IND forms you must complete all appropriate data entry fields. Click the Trainee Unique
ID field and enter the data. Select Educational Level/Highest Degree Earned dropdown to select
the degree for trainee. Select the appropriate degree. Select the Gender of the trainee.
Select the Age range of the trainee. Select the Ethnicity of the trainee. Select Race
of the trainee. Select Rural/Urban/Suburban/Frontier Background for the trainee if known. Select
the Disadvantaged Background status of the trainee if known. Select the Discipline/Specialty
for the trainee. Select the Trainee Status for the trainee. Enter the Type of BHPr Support
Financial Award (Multi-Select) code(s). Note: Use the MultiSelect Legend to enter the code
or code list. Enter the BHPr Financial Award Amount as a numeric value. Select the appropriate
answer to Program Completion for the trainee. Select the answer to the question, Did the
Trainee Complete the Program? Select the appropriate answer for the trainee. Select the appropriate
answer if known or applicable. Select the appropriate answer to the Degree or Diploma
question. Questions highlighted with a green heading represent faculty specific questions
and will be referenced on IND-FAC form later. Answer appropriately. Select the Promotion
Position if known. Use the MultiSelect Legend to find the appropriate code(s) for all the
Course Titles. Select the appropriate answer for this IND-FAC question. Select the second
row and continue your data entry until you have completed the IND_Template. Note: Remember
to save your data regularly to prevent data loss. Once you have completed the data entry,
save and close the file. You will then upload the file as you did with CE-1. Click the Save
button. Click the Close button. Click the Attach File button to upload the completed
IND-GEN spreadsheet. Click the Browse button. The Choose File to Upload window opens. Navigate
to the file location and click the File Name. Click the Open button. Click the Attach button.
The IND-GEN page appears with the file uploaded. Note: While the forms are listed separately
as IND-GEN and IND-FAC on the left side menu, the spreadsheet contains all this information
on one row for both forms. Click the Process Data button to upload the data from your completed
spreadsheet. This must be done to complete the data entry process. The Message from webpage
window opens, please read all messages carefully. Click the OK button to proceed. Click the
Save and Validate button. The IND-GEN form was saved and validated with no errors. In
this example the IND-FAC form will appear automatically since there are no validation
errors. You can see that IND-FAC has also been completed with the upload of the spreadsheet.
Select the Expand button to expand the left side menu. Once the left side menu has been
expanded you can see that the IND-GEN form shows that it has been completed. Because
the data for IND-FAC was completed during the upload process you may go directly to
the Save and Validate button now. Thank you for taking the time to complete this training
