if you have been selected for verification and you are still considered
dependent, you will need to complete the dependent verification worksheet and
submit it to the office of financial aid and scholarships
Be sure to fill out the form in its entirety,
including your student ID number.
This is different than your UC application ID number and your UCM
net ID.
It starts with the digits one zero zero.
Also be sure to include your last name,
first name, UC Merced e-mail address, and phone number.
Section A asks about your expected living arrangements for the 2013-2014
academic school year.
Select the option that best applies to you.
Section B includes questions about your household.
If someone in your parents' household received SNAP benefits, also known as food stamps,
anytime during the 2011 or 2012 calendar
year,
select this box.
Please note that the financial aid office may request documentation of the
receipt of these benefits,
so please be prepared to provide that.
If no one else in your household received SNAP benefits, skip to the next
question.
You will now list people living your household,
including yourself.
You, the student, should be listed first.
Other people who should be listed in this section include parents and their
children
who receive at least half of their support.
Be sure to include their full name,
birthdate,
their relationship to you,
name of accredited college attending, if applicable
and if they were claimed on your parents' 2012 tax return.
If any of these sections are left blank,
it may result in a delay in the verification process.
Section C asks about child support payments your parent has made during the
2012 calendar year.
Please note that child support cannot be listed as paid for children listed in
the household.
If this does not apply to your parent, list not applicable.
Be sure to re-enter your student ID number at the top of the page.
Section D involves questions about your 2012 income tax return.
Note that there are questions for both the student and parents.
If you have already filed and used the IRS data retrieval tool,
check this box.
If you have already filed your taxes,
but were unable to link your tax return
using the IRS data retrieval tool, select this box, and submit your tax
return transcript. This is different than the tax return and must be
requested directly from the IRS.

transcript.
If you did not file a 2012 tax return,
select this box and move to the "Non Filers Only" section.
Here, list the sources of any income earned in 2012 and the amount
earned.
Verification of income received, such as a W-2
or SSI statement
will need to be submitted with this form.
If no income was received in 2012,
put zero dollars.
Section E includes questions about additional financial information.
If the answer is the "zero" for any question or if a question does not apply,
enter zero dollars or not applicable.
If any question is left blank,
it may delay the verification process.
Students should fill out this form with their parent to ensure accuracy.
You can mail, fax, or bring your completed form to our office via the Students First
Center.
Due to the sensitive material that could be on your form, we do not accept e-mailed forms.
