0:07 “Hello everybody, my name is Dr. Jacobs, I am a Doctor of Physical Therapy.
I am also the inventor of Advanced Soft Tissue Release.
I am a cancer survivor and I used to suffer from chronic fatigue, migraines, headaches,
and pain before I invented the ASTR specialty.”
0:25 “Alright Gerardo, tell us what is going on with your knee?”
“Well I just had a meniscus repair and I am here for my first physical therapy.
Hopefully everything goes well.”
“Alright, when did you have the surgery?”
“January 3rd”
“So about a week or so ago.
So tell me currently, what is difficult for you to do?”
“Just bending my knee backwards, it feels tight.”
“So how much tightness do you feel from 0-10?”
“It’s about a 7 when I bend my knee”
1:06 “What else is difficult for you to do?”
“That’s it”
“But you were also saying when you walk a lot or sit and stand, you feel a little
stiffness?”
“Yeah, a little stiffness in the front of the knee.”
“And you mentioned that it is hard for you to put your socks on?”
“Yes, well 1 sock.”
“Okay let’s do this.
I want you to be in front of the camera and try to take your socks off so we can see what
is difficult for you to do and use it for the evaluation.”
1:45 “So bending it is uncomfortable?”
“Yeah”
“So you are not able to bend it and you compensate that way.”
“A couple days ago, I wasn’t even able to move it like that.”
“Okay so you are getting a little better.
And just bending, any other issues besides that?”
“No.”
“Okay, let’s take the other shoe off.”
2:25 “Now let’s have you walk back and forth to see how you walk.”
“Okay, now I’m going to take your range of motion and we’ll see how you feel after
that.”
2:55 “Alright so we’re going to take the range of motion here, let’s actually push
your knee down.
Alright so as you can see it is negative 10, almost 20 degrees.
Okay, bend your knee.”
“Put my foot flat?”
“Yes, foot flat and if you feel pain you can stop.
And as you can see here, it is 100 degrees flexion and you are missing 24 degrees extension.
Alright now let’s work on your knee and then after we will take the range of motion.”
4:10 “So this is your first treatment after the surgery right?”
“Yeah”
“And when you bend your knee, do you feel the tightness is the back, front, side?”
“The back actually.”
“Can you describe the pain?”
“Discomfort but tightness.”
“Okay let’s lie on your side.
So you feel it right here, is that painful what I’m doing?”
“No, it’s okay.”
5:10 “So mostly right here?”
“Yeah, right there.”
“Okay, so make sure it’s not painful.
Let me know if it’s too much.
And I’m going to
show you
the scar tissue in
a little bit, I am doing deep fascia release with this instrument.”
6:08 “Do you feel the tightness there?”
“Yeah”
“We’re going to test it with putting your socks on again.
It is very tight, you feel that tightness here?”
“Yeah”
7:00 “How does this feel now?
Can you feel the difference?”
“Yes, it’s looser now and movable.”
“Okay now I’m going to work on the scar tissue, make sure it’s not painful.
Do you feel this bump?
That’s the scar tissue.”
8:10 “So as you can see on the camera, it is nice and smooth up here, but rough down
here because the scar tissue is not allowing the instrument to move smoothly.
What I am doing now, is breaking down the scar tissue.”
“So with the treatment, you’ll notice that the scar tissue is going to shrink.
So when we have scar tissue, normally the muscle should act like a rubber band.
When we have scar tissue, it is like we have a knot in the rubber band.
So when the muscle retracts, it
limits
the range of motion.”
10:20 “See over here, it’s not too bad but down here it’s like jumping because
of the scar tissue and if we do not have a good ergonomic instrument to treat it, it
will be difficult to break the scar tissue by hand.
Can you tell the difference now?”
“Yes.”
“It’s totally almost gone.”
11:20 “Let’s take your range of motion again, okay bend your knee as far as you can
go.
And it’s very obvious, it’s 115 and how much did we say before?
Yeah it was 100.
Let’s see if we can get you a little bit more.”
13:00 “So I
am doing deep release to your fascia now, it should improve your range of motion.”
“You feel this band here?”
“Yeah”
“How does this feel now?”
“That feels good.”
14:10 “So all these ASTR instruments work on different soft tissue functions that would
be hard to get the same release with the hands.”
“Okay push your knee down.”
“Should I try to do it a little bit more, or just what I can do.”
“Whatever you can do and we’ll try it again.
Just relax and bend the knee.
There you go, 120 degrees.
Let’s have you sit on a chair to do the other test
of putting your socks on.”
15:40 “Okay now, we’re going to put your socks on and tell me how that feels?
Oh nice, you’re bending.”
“Yeah, before I had to bend it to the side a little bit but it’s almost normal now.”
“Okay, well what do you think about ASTR treatment, what we did in just a few minutes?”
“Felt great, I’m feeling great right now.”
