>> Hi. I'm Julie Rudolph with Educational Counseling
and Disabilities Services.
Today I would like to talk to you about our verified,
individualized services and accommodations or VISA form.
The VISA form is our primary form of communication
with you regarding a student in your class with a disability
who needs some classroom accommodations.
It is the student's responsibility
to provide you with this form.
We don't contact you directly,
but give that responsibility to the student.
We'd like them to sit down and talk with you
about the accommodations.
So, they should not give you the form at the beginning of class
or after class, but should actually make an appointment
with you or come to your office hours so they can sit down
and discuss the form with you.
There's limited information on the form.
Basically, we just want you to know about the accommodations
and how to implement them.
We don't provide any information about the disability itself.
If the student wishes to give you more information,
they can do that when they meet with you.
In some cases, we might email the form to you.
That would be if it's an online class or off-campus;
but normally the student will be meeting with you.
We do ask that you read through the form
in its entirety before implementing or working
with a student on any of the accommodations,
since there are instructions on the form.
On the form itself, you will see information about the student,
the semester, what class it is for,
and then you will see what accommodations the student is
asking for in your class.
Their initials should be next
to whatever accommodations they're choosing for your class.
It'll also provide information about the student's role,
your role as a faculty member, and our role
as disability services.
And then the form will go on to give instructions;
if it's extended test time or a note taker,
some specific instructions for you to follow.
