Complete all fields with correct information.
Click link on page or email to double-check information.
Print your permit immediately, from the website.
Within 15 days after event, mail form and payment.
BUT, if you're selling at a big event...
If so, your PROMOTER must provide you FORM ST-124.
THIS is the form to use: report and file sales receipts
Form ST-124 then IS your Temporary Sellers Permit.
