Dr. Spogen, you're a family practitioner at a patient-centered family medicine home. How
does this model help me manage my disease?
It really helps a lot. One of the things that we have in our patient-centered medical home
is a, what we call, a chronic disease registry.
The disease registry tells me evidence-based guidelines to follow, so I can keep up to
date on everything that you need to manage your diabetes correctly.
The other part of the patient-centered medical home is to help you focus on your disease
and to give you the education to empower you to handle your disease better.
It also involves a multi-disciplinary team. We will have nutrition specialists, exercise
physiologists, close at hand, so that they can help you manage some of the lifestyle
changes that you might need and help you manage your medications better.
So it's more than just the physician. It's a whole medical complex that are going to
help you get better.
