brief overview of the benefits available to UT System employees and dependents. We will also review benefits resources and steps to a
successful annual enrollment period. Keep in mind this is an overview. More detailed
information outside this presentation is available to you.
Refer to your UT Benefits Handbook for plan details and explore the Office of Employee Benefits website for vendor presentations and
contact information. Always feel free to contact a benefits representative at your local institution benefits office. During the
Annual Enrollment period of July 15th through July 31st, you should declare tobacco user status for yourself and your eligible
dependents. You may enroll in or change your group insurance benefit elections, add or remove dependents from your coverage
with or without a qualified change of status event, complete any Evidence of Insurability requirements, and enroll in UT FLEX programs.
All elections will be effective September 1, 2013 and will continue through August 31, 2014. If EOI determination is required for certain
elections, coverage will become effective upon approval.

Benefits eligible active employees have a basic coverage package.
For plan year 2013-2014 there are no changes or cost increases to the UT SELECT Medical annual deductible, coinsurance, or
copayments. There are no changes to the UT SELECT Prescription Drug Plan deductible or copayment amounts. There are also no
changes to the amounts offered for Basic Term Life Insurance and Basic Accidental Death & Dismemberment Insurance offered as part of
the Basic Coverage Package. The UT SELECT Medical Plan is administered by Blue Cross Blue Shield of Texas, Prescription Drug
coverage is administered by Express Scripts, formally known as Medco Health Solutions, and Basic Group Term Life Insurance and Accidental
Death and Dismemberment Insurance are provided by Dearborn National.
As an additional network option, The UT System Office of Employee Benefits – and The University of Texas Southwestern Medical Center are
continuing the collaborative health care pilot program called the UTSW Network during the upcoming plan year.
If you or a covered family member receives care from a UTSW physician or at a UTSW facility, your out of pocket cost will be greatly reduced or
eliminated. Benefits include a $10 primary care and $10 specialist co-payment, no out-of-pocket cost for inpatient or outpatient care, no
deductible, and no coinsurance for services offered by UT Southwestern. You do not have to be associated with UT Southwestern to be
eligible for this benefit.
The use of tobacco is one of the leading preventable
health risks worldwide. Because UT System is committed to promoting a culture of wellness and disease prevention while also recognizing
the costs associated with treating tobacco-related health conditions, the Tobacco Premium Program was
implemented for tobacco users enrolled in the UT SELECT Medical plan. This means you will need to declare your tobacco user or
non-user status for yourself and your eligible dependents during Annual Enrollment.
A tobacco user is a person enrolled in the UT SELECT Medical plan, age 16 and above, who
has used tobacco products within the past 60 days. A list of qualified tobacco products can be found on the Office of Employee Benefits website
For the 2013-2014 plan year, out-of-pocket premiums for full-time benefits eligible employees remains at zero for employee
coverage only. With dependents, the out-of-pocket cost has increased between $6 and $13 monthly depending on the level of coverage.
An out-of-pocket premium of $30 per month for UT SELECT members who use tobacco products will be applied to any tobacco user
enrolled in the UT SELECT Medical plan. This premium also applies to your spouse and dependent children. The premium for
dependent children is $30 per month regardless of how many covered dependent children use tobacco.
Meaning the maximum any family will pay is $90 per month if employee, spouse and dependent children use tobacco.
All types of tobacco products are included as part of the Tobacco Premium Program, including, but not limited to, cigarettes, cigars,
pipes, all forms of smokeless tobacco, chewing tobacco, snuff, dip, or any other product that contains tobacco.
Clove cigarettes, and any other smoking devices that use tobacco (such as hookahs). E-cigarettes, which contain nicotine, are also
included under the Tobacco Premium Program.
In addition to the Basic Coverage Package, active employees can select Optional Coverage, at an additional cost, for themselves and their eligible
dependents. Optional Coverage includes UT SELECT Medical with prescription drug coverage for eligible dependents, Dental, Vision, Long Term
Care, Additional Voluntary Group Term Life Insurance, Additional Voluntary Accidental Death & Dismemberment Insurance,
Short Term and Long Term disability for employees only, and the option to participate in UT FLEX. Flexible
spending accounts are offered for Health Care and Dependent Day Care expenses.
Here's what has changed in our benefits plan for this plan year.
Based on Internal Revenue Service requirements, the new Health Care Reimbursement Account annual maximum
election is $2,500. The annual maximum for the Dependent Day Care Reimbursement Account remains at $5,000 per calendar year if you report
as single or married filing jointly on your federal income tax return, or $2,500 if you report as married filing separately on your federal income
tax return. Also new this year – Everyone enrolled in the Health Care Reimbursement Account will receive a Debit Card at no cost.
You must make your UT FLEX Elections for both Health Care and Dependent Day Care for Plan Year 2013 – 2014 during this annual enrollment
even if you are currently enrolled in one or both of the UT FLEX plans. All UT FLEX annual elections will be effective September 1, 2013.
Beginning September 1, CNA will be offering a new Long Term Care plan to benefits eligible UT employees, retirees, dependents, and certain
other eligible family members. If you are a current Long Term Care enrollee, you may continue your coverage, but the current Long Term Care plan
will no longer accept new enrollees. Long Term Care insurance provides funds for necessary services when an individual becomes incapable
of caring for him or herself. The plan provides assistance in your home or care in a nursing facility.
The new plan offers a fifth daily benefit level at $250. Choose from either the Guaranteed Benefit Option or the Automatic Benefit Increase.
Some features of the new plan include: A 3-year lifetime maximum benefit instead of the 5-year lifetime max in the old Long Tern Care
plan. An additional daily benefit level option of $250. An annual Home Medical Technology benefit that will provide up to $1,000 per year for
monitoring and emergency reponse devices in your home and also covers home modifications to allow you to remain in your home.
And an annual Respite Care benefit that will provide up to 14 days per year to pay for Long Term Care services to relieve family members
and other ‘informal’ caregivers of their duties so they can take needed time off.
Current employees not presently enrolled in the Long Term Care plan will have a special opportunity to enroll in the new Long Term Care
plan during this Annual Enrollment without having to submit Evidence of Insurability. Employee’s spouses, and other eligible family members who
are not currently enrolled in the Long Term Care plan will need to complete the Evidence of Insurability information, and be approved for
coverage by CNA. All individuals presently enrolled in the Long Term Care plan may take no action and remain enrolled in the current plan with
the same benefits and premium rates as you have now. During this July 2013 Annual Enrollment period, if you are enrolled in the
Guaranteed Benefit Increase Option, you will have a special opportunity to buy up to a higher level of coverage without having to submit
Evidence of Insurability to prove good health status. In early July, all current Long Term Care enrollees will receive a personalized letter from
CNA describing your options and premium rates. For detailed information about the buy up option for the current Long Term Care enrollees, the
new Long Term Care plan, and your enrollment options during the July 2013 Annual Enrollment
period, call CNA Customer Service at 1-888-825- 0353.
There are no changes to the rates or to the benefits plan design for the following optional plans available to employees: Short Term
Disability, Long Term Disability Insurance, Voluntary Group Term Life Insurance, Accidental Death & Dismemberment Insurance, both the
Superior Vision and Superior Vision Plus plans, UT SELECT Dental, UT SELECT Dental Plus, the DeltaCare Dental HMO, and the UT FLEX
Dependent Day Care Reimbursement account.
Here’s how to make your elections during the annual enrollment period of July 15th to July 31st.

My UT Benefits is a secure website for which enrollment in coverage or changes to coverage can be made during the Annual Enrollment
period. My UT Benefits is also where you will declare your tobacco user or non-user status.
Login using your Benefits ID and pin, your UT EID and password, or your social security number and pin. At some UT institutions,
you can also login with your UT EID and password or, where Single Sign On (SSO) is available, by using your local Campus ID and
password. My UT Benefits is available 24-hours a day. New to My UT Benefits this year is the online dependent documentation system.

If you add a new dependent to any UT benefits coverage you must provide appropriate supporting documentation of eligibility such as
a marriage certificate, birth certificate, or other legal documents. The My UT Benefts online system allows you to
conveniently submit documents electronically when adding coverage for your eligible dependents during Annual Enrollment.
To do this, click the Dependents tabd. Simply upload clear, legible, digital images, scanned documents or photographs of required
documents directly through My UT Benefits as evidence of your dependents' eligibility.
Additional information will be available when you log into My UT Benefits
including the answers to Frequently Asked Questions about the documentation upload process.
After either declaring or updating your tobacco user or non-user status and making elections in My UT Benefits,
review your Benefits Summary. You will receive a confirmation letter via email summarizing your elections the following day.
Carefully review your confirmation letter – it will outline your insurance elections that will become effective September 1, 2013.
You will have until midnight, Wednesday, July 31st to make final decisions.
It is important to note: If you take no action, your current coverage (except UT FLEX Health Care and Dependent Day Care Reimbursement
Accounts) will continue for the new plan year. You will have to wait until next Annual Enrollment or a qualified change in status to
make changes to your coverage.

Attend your institution benefits meetings and fairs. Make your declaration as a tobacco user or non-tobacco user.
Make your elections no later than midnight CST, July 31st. Always, always review the Summary of Benefits before logging off the My UT
Benefits website. Complete and submit any Evidence of Insurability forms by August 15th, 2013. Provide Evidence of Eligibility
documentation for dependents by August 15th, 2013. It is essential to review your first pay check after the start of the new plan year
for accuracy. Let a benefits representative at your institution know of anything incorrect as soon as possible. There is a limited window of time to
make corrections. October 31st is the final day to ensure corrections are complete.

The UT System is committed to helping tobacco users quit by offering our UT SELECT covered members Smoking/Tobacco Cessation
Programs, Pharmaceutical Therapy and Nicotine Replacement Therapy (NRT) at no cost to the member. Learn more about programs available at

Wellness is so important that in February 2013 we launched a new platform to help you take an active role in the quality of your life, offering you a
reward for taking charge. The Office of Employee Benefits has teamed up with Provant Health Solutions, an independent health and
wellness company, to provide the new health platform to UT SELECT Medical plan members. This 24/7 comprehensive online health platform
offers wellness workshops, exercise routines, and meal plans. From the platform, email a dietician or ask a personal trainer a question.
You will have help identifying any health risk factors you may have, tools for positive choices to improve your long-term health, and a reward
for taking positive action. Members will receive a $25 gift card after registering at the platform site, completing the Health Assessment, and
completing your annual preventive exam all during this plan year. More information is available by reviewing the Welcome Brochure
(available in English and Spanish).
UT System offers two voluntary retirement savings programs, the UTSaver TSA (403(b)) and the UTSaver DCP (457(b)). These programs
allow you to tax-defer additional income for retirement, through pre-tax contributions. Enrollment for the UTSaver plans is available at
anytime of the year but Annual Enrollment is a great time to enroll or increase your contribution amount while you are reviewing your total benefits
package. The Office of Employee Benefits has partnered with five approved providers: Fidelity Investments, ING, Lincoln Financial Group,
TIAA-CREF, and VALIC.
