If you are on Medicare and believe that your provider has mishandled your care, you can file a complaint with AFMC.
To do that, you can go to qio.afmc.org to find the complaint form,
or you can call toll free 1-888-354-9100 to start the complaint process.
Once a complaint is filed, AFMC will consider your concerns, obtain medical records and review the complaint.
Our review is based upon widely accepted standards of care, and whether care you received meets those standards.
If AFMC finds that your provider does not meet those standards of care,
we will inform your provider and require them to take quality improvement measures.
If AFMC finds a serious problem, we may make a recommendation to the Office of the Inspector General.
The Office of the Inspector General will then handle the complaint and investigation and make any rulings,
which could include monetary penalties or sanction from the Medicare program.
