if you're anything like me filling out a form especially a form you're not familiar with
for the first time
well
let's just say i might rather be at the dentist than fill out a form for the
for the first time
that i'm not familiar with
it's not going to make my top ten list so anyways
thank you for taking the time to view this video
not understanding the new long-term care worker
...training requirements definitely can be confusing
and sometimes frustrating
in the next few minutes
I'm going to help you understand
how to fill out
the TPC form correctly
and submit the TPC form in a way that speeds up your approval process
now if you are a boarding home or an assisted living facility
or
if you're an adult family home wanting to fill out an approval form
or make application for approval
this TPC form right here
the one you see on the screen behind me to my left
this is not the form you want to fill out
so please again if you are an assisted living facility
or an adult family home
go back to the website where you found this form
and then you're going to want to click on the TPA form
again it's the TPA form
the reason for the TPC form
with initiative

DSHS is required
to approve all courses
curriculum
and instructors
for your training
The TPC form as you fill it out that's exactly what it's going to communicate
to us
the courses you want approved
the instructors you want aproved
and the curriculum you want approved once your training program is approved
you'll no longer use this TPC form to make any updates
you'll then move on and graduate to the TPC update form
the TPC update form

that you can click on
and review that or view that video as well
so let's quickly walk through the TPC form
there's five sections in the TPC form
however
prior to the first section
there's this little section right up here that really is self explanatory you're
just telling us the name of your business and giving us your contact information
section one this is where you communicate with DSHS which
long-term care workers
you plan on training
if you want to train workers from adult family homes you would check that box
if you want to train workers from assisted living
facilities you would check that box
so you can check all the boxes that you think may apply
section two
what courses you're planning on offering
to the long term care workers
If you'll notice in the table below
there's three columns the first column on the left is where you indicate
which course you're planning on teaching
the middle column is where you tell us the total number of hours for that
particular course that you're wanting to teach
and then the last column on the right is you're letting us know what curriculum
you plan on using
for that particular course
now i want you to keep in mind on the core basic if you click the core basic
training box
you're also going to want to put in your total hours but keep in mind that if you
are using enhancements
or enhancing the revised fundamentals of caregiving
your total number of hours is going include the twenty eight hours from the
RFOC
and then
you're gonna add whatever the number of hours that you're turning in for
enhancements
so it'll be twenty eight plus the hours and that will be here told me when you
put in this middle column
and keep in mind also that if you are using enhancements you want to send in
the CBRFOC form which can be found on our website
section three
instructors for
training
this is where you communicate to DSHS
what instructors you would like to be approved
and what courses
that you're wanting them to be proof to teach
notice in the table below
the far left hand column
is where you would put the instructor's name
first name middle initial last name
make sure you include the birth date as well
the middle column is telling us which courses they're planning on teaching
so just make sure you click on each box that's appropriate for that particular
instructor
the third column
is if this instructor is approved to teach
this course or one of these courses from another DSHS approved training
program
you mark yes in that box and let us know which training program
that is
now the orientation or safety
or
continuing education
if you're applying to teach any of those courses you do not need to submit an
instructor application
you'll only fill out the attestation section which will get to in a moment
all the other courses do require
that you fill out and submit
instructor application form
section four
instructor attestation for orientation
safety training
and CE instructors
by completing section 4 you are verifying
that all the instructors listed in section three
that are planning to teach orientation or safety or continuing education
meet the qualifications to teach that
if you are the person submitting this form you want to put your name
your job title
and the date
in this section
section five
getting information to the department
the department being DSHS, that's us so section five is going to tell you
what you need to turn in and really the best way to turn that in and the
most
well right here says how to streamline the approval process

so that you know if your application is complete
