After breast augmentation, the ideal position of the lower portion of the breast implant
is at the level the inframammary fold.
In sub-muscular placement,
the implants are below the muscle and the muscle originates at the level of the clavicles.
Because of this, and the fact that the muscle is elastic, the natural position for the implants
to sit immediately after augmentation is midway between the inframammary fold
and the level of the clavicles.
In this position, the implants are actually higher than the breast themselves and we refer
to this is high riding implants.
Ideally, we'd like to move the implants down so they would fit within the breast envelope.
This can be achieved after surgery by using a device called a Bandeau which helps to stretch
the lower pole skin of the breast.
I'd like to show you the correct positioning of this Bandeau device.
Shown here the Bandeau is covering the upper portion of the implants and this is actually the wrong
position.
Ideally, you'd like the Bandeau placed even higher up to the level of the clavicles,
so that it's above the level of the implants.
In this type of placement, it will actually help push the implants down into the ideal location.
Seen on a profile view,
the improper position of the Bandeau would be shown here,
covering the upper portion of the implant and pressing on the breast without really pushing
down.
The ideal placement is actually higher
up to the level of the clavicles,
so when the Bandeau is placed low in the back (at the level the bra strap) and high on the
front near the level of the clavicles,
it'll end up creating a force that is predominately pushing downwards
and this will help stretch the lower pole skin of the breast out
and help the implant drop into position.
