by John Hancock. Unforeseen events can occur at anytime in your life. Long-term care insurance
provides coverage for necessary care provided outside of a hospital setting, such as a nursing
home, in your own home, or in an adult day health care facility. You may apply for this coverage
at anytime; however, if you apply within the first 31 days of the date you become eligible for
benefits, proof of good health may not be required. Answer the qualifying questions to determine
if you need to submit proof of good health. Submit your application directly to John Hancock.
Your eligible family members may apply, even if you choose not to. Your spouse,
state-registered domestic partner, your parents, and your parents-in-law are all eligible
to apply for the long-term care insurance.
John Hancock offers employees three long-term care options, the Bronze plan, the Silver plan,
or the Gold plan. The plans offer coverage for nursing home stays, alternate care facilities,
home care, adult day health care, and informal care. Each plan has a different daily maximum
amount and a different lifetime maximum amount.
