Hi. My name is Dr. Nissan Pilest. I'm a board certified dermatologist. And today I would
like to discuss the use of Cosmelan and Cosmelan in the procedure. This lady, her name is Mariah,
and as you can see on her face she has a number of different pigmentary changes problem, essentially
on her mid forehead here, she has some melasma.
She has a lot of pigmentation all over her cheeks, her chin, her nose. Melasma by itself
is actually a condition, it's a disease, and it's usually seen in women, because of use
of birth control pills or having children and also being in the sun. There is two kind
of products. One is called Cosmelan 1 the other part is called Cosmelan 2. Cosmelan
1 and Cosmelan 2 contain different ingredients, but essentially they are mostly bleaching
creams, some exfoliators.
Once we put this on the patient's face then we leave it on anywhere between eight and
twelve hours and then the patient will wash this off and then she will use Cosmelan 2,
two or three times a day depending on her tolerance for a period of one to three months,
and of course she will also use a sunblock. Before we do Cosmelan this package comes in
with a special degreasing fluid that we will be actually using on her skin, we'll take
all the oils out any makeup or anything.
We'll of course wash her face before and remove all make-up that we do this as well if there's
any excess that would come off as well. And we'll make the skin pretty ready for us to
apply Cosmelan. So this is the Cosmelan 1 and we are going to use all of the content
of this little jar on her.
So we're almost finished. We tried to put extra layer or two on the area where we had
the melasma. Make it maybe a little thicker application there. And that's primarily on
her cheekbone area and the malar area, right here, so that we put a little bit more also
on the upper lip. She had a lot of pigmentation there.
I'm gonna put some extra. This is really the entire procedure. We'll ask her not to touch
this just to go home and stay home for 8 to 10 hours. She can go up to 12 hours if she
wants because she is about for skin so she can tolerate this a lot longer. Well, we ask
her if there is any irritation or any itching to go ahead and take off that area that itches.
can be repeated, 3 to 6 months later, again using the Cosmelan 1, followed by use of Cosmelan
2 at home, or she can just continue using the Cosmelan 2 once she gets much better for
control. And she will use the Cosmelan 2 MD, which is the one that has retinae and hydroconoyl
on either once every night or once every other night on ongoing without any problems for
as long as she wants.
We don't use this product on patients who are nursing or if they are pregnant. Otherwise,
there is no other contradiction to the use of this product. Thank you.
