more than one-third of US adults are obese at the cording to the Centers for
Disease Control and Prevention and a small minority in adults and even now
teenagers
are taking matters into doctors' Hansen opting for weight loss surgery
on Monday March 24th at 10 p.m. independent lens
well-run a documentary titled all at me a story of love
lost and last resorts about a group a friend to opted for this way losing
weight
and joining me today to discuss this is doctor john Romanelli
medical director up weight loss surgery at Baystate Health
thank you so much doctor welcome for joining me so let's talk about this
becoming a
Anna on say a new fad but seven coming in new way of handling
I'll weekend is it safe
it's definitely safe I would say its new though
so surgery for the purposive causing weight loss dates back to 1954
I and gastric bypass which is today Kat the most common or most well-known
operation if you ever hear stomach stapling that's generally what they're
referring to
that dates back to nineteen sixty eight years and then in
Iowa New Jersey Iowa so surgery for weight loss
isn't new what is new is the public accepting
that obesity is a disease and should be treated as such
and I think that's new in the last five to 10 years so
doing something about obesity in terms of surgical management obesity has
become far more common because as much more acceptance
both from public standpoint and from the pair standpoint may have people
kind on the opposite side defensively say no it's not a disease you can do
something about it
but it me are you that know you can sometimes
it does happen you have to resort to medical procedures
why I've treated thousands have morbidly obese patients in my career and I can
tell you I'm still waiting for the first one
to show up to tell me that day wanted to be that way
so that misconception that you could do something about your weight
when you're morbidly obese is really not true because there's something different
in the neurological wiring up obese people think that is a thermostat that's
broken and it's set
too high and so is hot in your house as the he's blasting in
well turn often told eighty degrees in your house well
the setpoint for when patients feel follows different robie's people
in other words you when I E a small amount of food
and we're satisfied not hungry until the next mealtime but for morbidly obese
people they need significantly higher should violence to achieve that feeling
in some never do it all and that's where the procedure
comes into effect it right so operations for weight loss surgery are designed to
allow you to eat smaller volumes and food and therefore taken less calories
per meal
yet not be hungry half an hour later need to be hungry again at the next
normal mealtime
so it's an attempt to reset the thermoset so to speak
stetson at a normal volume of food what we call normal is
maintaining our are healthy weight a normal amount of food
will make them satisfied and not hungry until the next meal now
this procedure start and become more common and teenagers
yes talk about that well so let's let me clarify before we get to the teenagers
there are three different types of procedures there's gastric bypass which
I mentioned
there's gastric banding more and and there's two different devices but the
common trade name used as the lap dance and its people heard that
product and then there's a newer operating costs sleeve gastrectomy which
in the course at three or four years has emerged as the most popular operation
that's the operation will perform on teenagers arm sleeve gastrectomy is
elegant in its simplicity you want to eat less
I me stomach smaller quite literally all we do is reduce the size in the stomach
to about one-third the normal size and stomach
and shaped like a sleeve said the name is one the few things and certainly not
named after its inventor
but the stomach become shaped like a sleeve or narrow tube
and sell it can't bill for food now it's
me normal stomachs the stomach distension stretches
bills for food miss lee can't do that to the patients get fall
very rapidly and Nate feel fall
does that negatively impact the rest of the body
by having a different sized you're changing the size of the stomach
you are but no it does not it's very safe
and safe enough to now do on teenagers credit is
and the thing is with both gastric bypass in gastric banding there were
reasons to think about doing in teenagers
you have to understand that in the adolescent which in my opinion in this
is no disrespect to any care providers bears a relatively
underserved part of our population getting a little old to go to the
pediatrician too young to go to a primary care doctor
and so by adolescent I put anyone from like 16 to 25
in that group really and if you think about yourself or
me you know you go to college graduate cut my boxer
are you have a primary care doctor yet if you're in a community with family
practitioners
we have the same care provider from childhood to adulthood
that's different but that's not everywhere
so these patients are serves
period now add to obesity you have somebody that CCP efficient usually
through age 18 or UGC and a high school
might be overweight has some obesity problems they then go through the
college years or their early work years
and they don't really find a doctor until they have a problem
somebody's people are going to a doctor and other 30 35
and it's a real problem and the reality is they already have a problem that'll
be /c
obesity is a disease it's a medical problem that requires treatment
and it the idea I've operating on these peoples
why wouldn't I have diabetes why wait till you have high blood pressure
take care the weight problem as the primary problem before they ever get
sick from it
and prevent them from ever getting there would you consider this a quick fix
or truly a long-term through absolutely not a quick fix
on it weight loss surgery is a tool
it is a tool that can help people lose weight in conjunction with diet
in conjunction with regular exercise and behavior modification what what does
that mean he modification means changing the way we think about
food back eating about your lifestyle means
getting rid the sedentary lifestyle means exercising regularly
which can be a challenge for people are significantly overweight which means
they need help
might mean an exercise physiologist to design an exercise program for them
it might mean that okay they can't walk on treadmill because their knees and
hips and feet her all day
but in a swimming pool they might be able to burn significant amount of
calories
so weight loss surgery by itself does not
cure obesity it is a tool to fight obesity
and when used correctly with dieting with exercise
with behavior modification can be very effective
great thank you so much aroma now I from basic health
