If you’ve been in a motor vehicle accident in New York State that resulted in more than
one thousand dollars of property damage, personal injury, or a fatality, you have to fill out
an MV-one-“o”-four form to report your accident to the Accident Records Bureau.
Under New York State law, you only have ten days to complete this form and get it filed.
If you don’t, you could face misdemeanor charges and a suspended license and registration.
So don’t wait—fill out your MV-104 form as soon as possible. You can pick up a copy
of the MV-104 form from our office, most police stations, or your insurance company. In addition,

looks similar to a police report. The difference is, the police are responsible for filling
out a police report at the scene of the accident—the MV-104 form is your responsibility. You can
use the police report as a guide for filling out the MV-104 form, because the information
needed for both reports are similar. The MV-104 form gives each driver the chance to document
the accident. After you’ve read the instructions on page
two, fill out all the contact and accident information on the report and fill in the
numbered boxes. Remember, only use black ink and always enter your information as “Vehicle
One.” You don’t have to use too much detail when describing the accident, but it is important
