Adrian Richards: Hello. Our patient today had 295
cc ultra-high PIPs inserted back in 2004. She's quite happy with the size, would like
the PIPs replaced, and if anything, would like to go slightly fuller, and so I'm going
to be replacing the implants during this video. If you'd like more information about PIP implants,

Our patient has got implants. She's happy with the size. The scar was a bit tethered,
so I'm going to release that a little bit and replace the implants with a very similar
size implants, because she's happy with the size.
The first stage is to remove the scar, and then I'll show you the implants as they're
removed.
I've just opened up the capsule around the implant. Can you see this creamy material
coming out? That implies that the implant is ruptured. I'll have a look. I'll get the
implant out and I'll show you in a sec where the rupture is.
I've removed the implant on the right and you can see it's really quite severely ruptured.
It all seems to be intact, but we've got a severe rupture right the way through the top
of the implant there. The next stage is to clean everything out and replace the implant.
I can see immediately this implant is ruptured, as well. Can you see the silicon just coming
out there? So, we've got bilateral ruptures on both sides, with the silicon gel bleed
and the fluid. So I'll show you this implant in a second, as well. I just need to get it
out intact, well as intact as I can.
...implant I've removed. It's got a similar sort of rupture round the base plate here.
It's a common place for them to rupture. Very, very standard rupture. This is what we're
seeing, a lot of patients with ruptured PIPs. This sort of appearance.
I'll show you the implants on both sides, both ruptured actually in quite similar places
if I just orientate them the same. Can you see? This one's ruptured here at the base
plate, this one's ruptured here at the base plate. This is the base plate, the round circle
at the back, and you can see both ruptured similar positions there. Different lot numbers
and batch numbers, but we can see both implants ruptured in a similar fashion, as you can
see.
