Today we will look at the nine pages required for your intent packet to make sure your project
is approved so you can get to work!
Notice on this very first page, it�s our project approval form this is the score sheet
to be completed by the approval committee. We are asking you to submit it, it is the
first page of your packet. All you need to complete, however, is this very top portion.
Note we are asking you for the title of your topic as well as the industry sector. Every
project will fit within one of the California industry sectors. If you�re not sure which
one applies to your project, then you need to Google �California Industry Sectors�
and determine which one is appropriate.
So this is the cover sheet for your project paperwork. This is where the committee takes
a look at each of the criteria and determines whether or not your project will be approved.
Hopefully they will sign off on it, your principal will sign off on it, and you will be good
to go!
Now I�m going to go through these next pages pretty quickly, but copies will be available
on the internet for you to look at if you would like to take a little more time and
look at some of the details.
We expect you to complete this form with your mentor, the Expected Outcomes form, and make
sure you complete every section that you are supposed to, using complete sentences and
proper punctuation and grammar. So we are asking you to complete his entire sheet which
is basically an outline of what your project is. It tells us everything we need to know
so we can determine if it is going to be a successful project. You need to sign off on
this as well as your mentor.
The next sheet is the tasks and timeline form, again, completed with your mentor. And what
this does is take the entire senior project and breaks it down into manageable chunks,
which we are calling steps or tasks. We ask you to identify each one and estimate how
many hours are required and what your anticipated completion date is. This will help make sure
that you stay on track. This is your road map to success! Time Management is what it
is all about. Note, you sign off on this form as well as your parent and your mentor.
The next form is your Anticipated Costs form. Some projects will have no expenses, and that
is fine! You will simple put �no anticipated expenses� and a zero here and a big zero
here. If there are anticipated costs involved go ahead and estimate what they are, and be
sure and tell us how you are going to fund your project. VERY IMPORTANT. Sign off on
this, have your parent sign off on this, and again your mentor will sign off on this sheet.
Next we have the mentor contract form. All you need to do with this is complete your
name and the name of your topic. The rest of it is up to your mentor. They�re going
to complete this form and tell us why they are qualified to be a mentor, and give us
their contact information in case we need it, and sign off that they are willing to
serve as your mentor. VERY IMPORTANT.
This sheet is your Job Shadow Partner Contract, similar to the last one, however this is for
your job shadow, making sure that you have identified how your job shadow is and you
have contacted them and you have dates for your interview as well as your job shadow.
They can be the same date, but it doesn�t need to be. Make sure you sign off on this,
as well as the professional you have chosen as your job shadow partner, and date that�.
This is your Parent Consent Form, your parent needs to sign off that they have read and
approved all of these forms. Make sure that your name is here, and the project topic and
outcome is up here. Then make sure your parent signs off on this and you do too. This way
we make sure there are no surprises with the senior project!
This particular form just makes sure the school district isn�t held liable in case something
happens. Just be careful! Make sure your name is here, the name of your high school is here.
And basically you understand and acknowledge that doing this, you could get hurt � but
please don�t. You have to provide medical insurance information here. Your parent or
guardian signs here, and then you sign off on this form as well.
Very last form is the Student Contract! Read through it very carefully! Note what you are
agreeing to! Completion of your project, and by what date! It also includes a little statement
about plagiarism. You want to make sure the work you do is your own. Sign off on this,
have your parent sign off on this, and then submit the information electronically to your
instructor. Hopefully you get it approved and you are good to go.
The way you get it to your instructor: Print it off, complete it, get your signatures,
scan it, and then email it or upload it for your instructor.
